  • Joseph Brant Hospital is currently seeking a Stroke Coordinator. Reporting to the Manager, Inpatient Rehabilitation, the successful individual will be accountable for the facilitation, implementation and sustainability of patient-centered, stroke best practices at Joseph Brant Hospital, across the continuum with a focus on hyperacute, acute care, and post acute care. This will involve continuously providing clinical and professional leadership, raising awareness, providing education, informing models of care delivery, building capacity in others, managing projects, monitoring data and conducting evaluations. The engagement of multiple stakeholders in developing or implementing best practice tools and resources will be required. This role is ideally suited to an individual with strong clinical knowledge and leadership skills who will promote the delivery of safe, quality patient care, facilitate staff development, and maximize the efficiency and effectiveness of the teams related to stroke care.

RESPONSIBILITIES:

  • Participate in evaluation design, monitoring, analysis and change management. This includes program monitoring and evaluation to identify best practice gaps including inefficiencies; planning and monitoring continuous improvements in practice. This includes the implementation of Accreditation Canada's Stroke Distinction program.
  • Assist and advise on proposals and business cases that will address care gaps. Investigate innovative approaches or programs to improve health outcomes including system integration opportunities for better efficiencies. Inform strategies to sustain and improve performance.
  • Identify and share methods to operationalize new best practices. Review research, qualitative and quantitative evaluation findings to identify gaps in care and knowledge. Assist in identifying priorities and participate in quality improvement initiatives including knowledge translation activities.
  • Represent JBH on several Central South Regional Stroke committees and translate information from these committees into practice at JBH
  • Development of Stroke Education/Resource for Patients and Staff including all members of the interprofessional team
  • Responsible for ensuring Stroke Data Quality
  • AlphaFIM, OTN Telestroke & Stroke Research Site Coordinator and JBH/MOD After Stroke Peer Support Volunteer Visitation lead

QUALIFICATIONS:

  • Four (4) year Undergraduate Degree in a health related discipline; Masters Degree preferred
  • Member of a regulated health profession under the Regulated Health Professions Act with active member status
  • Minimum five (5) years recent stroke clinical experience
  • Demonstrated advanced skills as an educator and knowledge of adult learning principles
  • Expertise in stroke care and knowledge of the continuum of stroke care
  • Clinical experience working with an interprofessional team
  • Previous experience as a change agent with an understanding of organizational cultures
  • Demonstrated knowledge of data collection, use of databases, data analysis, and evaluation
  • Advanced interpersonal and communication skills (both verbal and written)
  • Demonstrated advanced project management and leadership skills with the ability to manage multiple projects effectively
  • Demonstrated presentation skills and ability in the planning, implementation, and evaluation of educational offerings
  • Ability to design, appraise, and/or evaluate educational programming appropriate for stroke heatlh care providers working in practice settings across the continuum of stroke care
  • Ability to understand, critically appraise healthrelated research and the ability to transfer and translate knowledge into best practices and relevant recommendat
