Case Manager - Supportive Housing (Abode, Forward, Housing Retention)
Reporting to the Manager, Community Programs, the Case Manager, Supportive Housing is a social worker or equivalent, who provides supports to individuals and families in the supportive housing and housing retention programs to achieve community integration while working on a long-term plan to achieve housing stability in the community.
What does a Case Manager do?
- Coordinate independent life skill case management sessions to promote independent living and progression into independent community living.
- Conduct goal planning and regular checkins with individuals including but not limited to home checkins.
- Orient individuals to landlord and tenant rights.
- Support eventual permanent housing relocation by the end of the program.
- Provide individual supports to identify gaps in health and independent living skills including but not limited to landlord tenant negotiation, selfadvocacy, and budgeting and life skills and providing referrals as necessary.
- Facilitate informal brief intervention counselling to promote independence and building healthy partnerships for success.
- Keeping up to date with new and current communitybased programs and support.
- Appropriate documentation of client meetings, preparing reports on monthly and quarterly statistics.
What you bring to the table:
- Minimum Bachelor of Social Work Degree or equivalent degree or diploma plus case management experience
- Knowledge of Landlord Tenant Act.
- Knowledge of Housing First Practices.
- Experience working with vulnerable populations.
- Must have a reliable vehicle, valid driving license and appropriate insurance. Mileage is reimbursed.
- CPI, CPR and Vulnerable Screening Clearance (Police Check).
- Experience with harm reduction practices.
Salary information:
$25.22 per hour

Education:
- Bachelor's Degree (required)
Experience:
