Job Summary

Within the Department of Family Medicine, the Queen's Family Health Team (QFHT) is a multidisciplinary team of health care professionals who provide primary health care services to the Kingston and Belleville communities.

As an academic teaching centre, the Department trains future family physicians and conducts research in issues of primary health care delivery.

QFHT is comprised of a number of medical practices and provides for a diverse, client centred work environment for all its employees.

QFHT provides primary health services to rostered patients while modeling safe, efficient and collaborative clinical processes to Family Medicine residents and other health professional learners.

The Data & Quality Improvement Analyst is a key element of this objective, helping to support the delivery of high quality care, as well as education and research programs in this area.


JOB SUMMARY:


Reporting to the QFHT Clinical Operations Manager, the Data & Quality Improvement Analyst will provide complex data analysis, systems support, representation of the QFHT at local, regional, and provincial data-related discussions, and other administrative duties in support of the DFM and the QFHT.

The incumbent's responsibilities include the extraction of data from an electronic medical record (EMR); presentation and analysis of data for a variety of audiences, including physicians, clinical and clerical staff, researchers, management and external stakeholders; coordination and support for quality improvement initiatives; and implementation of data discipline processes.

This position will require the incumbent to keep current with the EMR functionality, changes to the EMR database and the report functions within the system.

The incumbent will be expected to contribute knowledge of quality improvement and research methodologies, data management, privacy and data security standards.

The incumbent will contribute to the education of family medicine residents/learners and other professional staff within the clinic through regular educational updates and evidence-based research.

The dynamic environment of primary health care data collection and reporting, requires the Data & Quality Improvement Analyst to participate in several organizational, local, regional and provincial communities of practice and committees.


Job Description:


KEY RESPONSIBILITIES:


Data Analysis:


  • Perform detailed (MySQL) query development and report specifications and/or development to support business and clinical requirements and information needs.
  • Conduct analysis, prepare reports, data summaries and presentations and present this information to a variety of audiences, including the Ministry of Health and Long-Term Care, senior management and frontline staff to increase their awareness of data governance, information delivery and quality improvement efforts. Regularly provides status updates concerning key data points to stakeholders.
  • Collaborate closely with interdisciplinary staff to identify performance metrics and data quality needs, specifically in terms of program development and evaluation.
  • Conduct additional data analysis to identify missing or erroneous data in the EMR database; makes corrective and proactive recommendations to improve data integrity, including process improvements, training tools, and IT solutions.
  • Seek out external sources of relevant data and information to DFM and the QFHT and strive to maintain access to that data, analyze and disseminate it.
  • Ensure that QFHT stakeholders remain uptodate on related data governance strategies and practices, tools, leading or emerging trends and issues in the health care sector.

Systems Support:


  • Work with the ITS Administrator and others to establish an appropriate Business Intelligence and reporting framework for the QFHT.
  • Maintain a digital connection and accounts with external organizations in order to fulfill the QFHT's data sharing agreements and other informational needs.

Representation of the QFHT:


  • Collaborate and share work product with Quality Improvement Decision Support Specialists (QIDSS) and other professionals working at Family Health Teams across Ontario, including the Association of Family Health Teams of Ontario (AFHTO).
  • Engage and collaborate with local decision support professionals in other healthcare organizations (e.g. Kingston Health Sciences Centre, South East Local Health Integration Network, Kingston Frontenac Lennox and Addington Public Health).
