CONTINUED CARE & SUPPORT FOR YOUTH WORKER, TEAM 8

CHILD & YOUTH SERVICES
To provide support and guidance to assist youth, up to age 21.

When working with youth, who are transitioning from the care of the agency to independence or a supportive program, a youth-centred and strengths-based approach is used to support youth to achieve physical and emotional wellbeing, acquire basic life management skills, and develop social networks that include connections to caring adults and the community.


MAJOR RESPONSIBILITIES

  • Provides ongoing support and counselling, as well as crisis intervention, and assesses the client's strengths and needs in a variety of contexts and makes recommendations as necessary.
  • In collaboration with transitioning youth, identifies the specific strengths and needs of the youth and in accordance with Ministry Directives and agency policies, ensures the effective development of the CCSY Agreement and Youth Plan.
  • Reviews the plan with youth as required and at a minimum of once every 3 months.
  • Works with the youth to develop enduring and permanent relationships to support their transition to adulthood.
  • Identifies specific agency and community supports required by the youth in order to build on their strengths and meet their ongoing needs.
  • Prepares and completes all written requirements in accordance with Ministry Directives, and agency policy and practice procedures.
  • Advocates for youth where necessary in areas such as education, employment programs, community agencies, housing, etc.
  • Participates in relevant case conferences to contribute to service recommendations.
  • Meets regularly with supervisor as contracted for review of cases, consultation and professional development and keeps the supervisor informed of all situations which may be problematic in nature such as uncompleted work or crisis situations.
  • Participates in child welfare court, and criminal and youth courts to give evidence and support the youth.
  • Attends and participates in regularly scheduled team meetings, by seeking out and providing feedback in case consultation, individual support, program development and team cohesiveness.
  • Participates in ongoing training as necessary to stay current and up to date with the demands of the work.
  • May supervise C.Y.W. students or students in related disciplines.
  • May assist in transporting clients to appointments.
  • Works flexible hours as required for identified needs/goals for the youth
  • Works in a safe manner in accordance with the Society's health and safety policies and procedures, and all relevant legislation.
  • Acts in accordance with and incorporates Society's Code of Ethics, Confidentiality, Anti-Oppression/Anti-Racism, Harassment & Discrimination policies, etc.
  • Uses sound judgment in consideration of financial resources.
  • Complies with Society's financial policies and procedures.
  • Performs other tasks and assignments as requested or required by the supervisor.

QUALIFICATIONS

Education and Experience:


  • Certified Child and Youth Care diploma (formerly Child and Youth Worker diploma) plus a minimum of 3 years relevant experience, or
  • An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job.

Knowledge and Skills:


  • Demonstrated knowledge of the impact of trauma on youth and young adult development.
  • Indepth understanding of the experience of being a youth in care.
  • Demonstrated understanding of the issues that youth from care experience as they transition to independence.
  • Previous experience and/or knowledge of support services for young adults such as housing, health care, mental health, addictions, court services, etc.
  • Demonstrated skill in assessment and diagnosis, setting goals, implementing service plans, contracting and advocacy for clients.
  • Demonstrated ability to effectively problemsolve and manage crisis situations.
  • Experience and competence in a variety of service models including: individual, dyad, family counseling and group work.
  • Demonstrated ability to work collaboratively and effectively within the program, across disciplines in the agency and with community resources.
  • Excellent organizational and time management skills and the ability to work independently.
  • Excellent oral and written communication skills.
  • Strong interpersonal skills are essential as well as the demonstrated ability to carry out responsibilities with flexibility and creativity.
  • Demonstrated ability to work cooperatively, negotiate effectively and facilitate teamwork.
  • Proficiency in computer software (e.g. Microsoft Office, Sharepoint, CPIN)
  • Valid Ontario Driver's license.
  • Able to drive on a daily basis including long distances.
  • Access to a vehicle for work purposes
  • Proficiency in a second language, preferred
  • Experience with court proceedings, an asset
