Position Summary:

This position will report to the Director of Tenant Nursing.

This position will support Director of Tenant Nursing to plan, organize, implement and evaluate all activities of the assigned care and services for the Assisted Living Services for High Risk Seniors Program clients in order to provide high quality care.


Major Responsibilities:


  • Manage the administrative functions and daytoday operations of the Assisted Living Program, including care planning, updating Service Records and completion of Community Health Assessments (CHA) and Screener Assessments.
  • Enters all demographics, assessments and all other relevant tenant information into PCC.
  • Utilize PCC to develop and create and keep current the Tenant Service Plan / Care Plan.
  • Initiate POC tasks in PCC for PSSW staff to utilize, maintain and keep current.
  • Audit the effectiveness of staff utilization of the POC tasks in PCC.
  • Effectively plan, organize, implement and evaluate services in accordance to legislated requirements.
  • Advise the Director of Tenant Care (DTC) on relevant administrative, care and service issues.
  • Provide efficient management and highquality patientcentered service delivery.
  • Facilitate interdisciplinary processes and team functions.
  • Work in close collaboration with the Director of Tenant Care and Home and Community Care (HCCS) regarding the wait list, admission criteria, eligibility of clients and discharge planning.
  • Implement communication systems that will facilitate communications regarding clients' status service needs and care delivery.
  • Performs other duties and responsibilities as assigned by the direct supervisor

Qualifications:


  • At least 3 years' experience in a community health care environment.
  • RPN certification and in good standing with the College of Nurses
  • Experience using PCC, and knowledge of RAI-CHA is an asset
  • Knowledge in the field of gerontology and dementia.
  • Excellent interpersonal and communication skills.
  • Must be able to accept responsibility and work independently.
  • Maintain valid cardiopulmonary resuscitation (CPR)Level C.
  • Evidence of good physical and mental health.
  • Ability to demonstrate diplomacy and professionalism when working with families and other professionals.
  • Exceptional interpersonal and customer service skills
  • Demonstrated success in communicating with all levels of the organization that includes communicate effectively with staff, tenants, families/visitors delivery/service personnel and the general public.
  • Excellent problem solving, conflict resolution, and decisionmaking skills.
  • Good judgment skills and the ability to handle and maintain confidential information
  • Ability to multitask and prioritize workload.
  • Ability to work independently as well as function as part of a multidisciplinary team.
  • Ability to be proactive and take initiative where required.
  • Ability to focus on assigned tasks
  • Detailoriented and professional
  • Ability to effectively handle multiple tasks simultaneously
  • Ability to work under conditions of frequent interruption, being undisturbed by the complexities and variety of minute details.
  • Demonstrated ability to observe boundaries, engage in appropriate emotional regulation, refrain from dual relationships with tenants, maintain confidentiality, and engage in reasonable selfcare strategies designed to reduce stress by balancing work/life responsibilities.
  • Ability to read, write and speak English.
  • A satisfactory and current Vulnerable Sector Screening.
  • Clear 2 Step TB Test and Flu Immunization
  • Full-Covid19 vaccination, and Booster dose (recommended unless medically exempt).
