JOB SUMMARY:


The Home Management Consultant is responsible for helping families of children with an intellectual disability cope with challenges by providing in-home consultation and collaboration with other professionals to plan for services.

Delivers in-home consultations to assess and provide skill-building, informal counselling, and behaviour consultation designed to enhance a family's ability to meet the needs of a child with developmental disabilities and prevent family breakdown.


QUALIFICATIONS:


  • Bachelor's degree in Social Sciences or a related field.
  • Postdegree specialized study in behavioural science.
  • A minimum 5 years of experience conducting assessments, designing intervention strategies, and working with families and children with developmental disabilities.
  • Experience in crisis intervention an asset.
  • Knowledge of family systems theory. Knowledge of child development.
  • Knowledge of behavioural strategies and strengthbased approaches.
  • Knowledge of various relevant diagnosis and medications.
  • Understanding of individual rights and ethics.
  • Understanding of how to develop Person-Directed Plans.
  • Proficiency in computer software, including Microsoft Office (Word, Excel, PowerPoint, Outlook).

RESPONSIBILITIES:


CONSULTATIONS:

  • Delivers inhome family consultations providing inhome behaviour, family, and skillbuilding assessments and program strategies to enhance individual and parenting capacity based on individual goals.
  • Advocates on behalf of families, acts as liaison, and collaborates with internal and external resources and supports to enhance inhome programs and strategies.
  • Handles requests for Person-Directed Plans from Developmental Services Ontario (DSO).
  • Coordinates, facilitates, and develops plans which help individuals with a developmental disability set goals, make choices, find ways to participate in their communities, and identify resources they can access.
  • Coordinates provision of necessary external supports and services to assist families and facilitate progress of child and family toward specific goals.
  • Assesses the effectiveness of strategies and determines when to make necessary changes.
  • Makes determination regarding when to say no to a family, or when to seek out other resources and external supports for an individual and their family.

ADMINISTRATION:

  • Reconciles and submits monthly travel expenses for reimbursement.
  • Monitors and tracks Ministry of Children, Community, and Social Services (MCSS) attendance as required by agreements.
  • Inputs data and case note updates to the ISIS client database and maintains all clientrelated files.
  • Completes all mandatory online training in a timely manner.

TRAINING AND SUPERVISION:

  • Develops and facilitates internal and external training workshops (e.g., rights, family systems, bereavement, sibling, sexuality, parenting).
  • Supervises placements for students completing Bachelor of Social Work and Master of Social Work programs.
  • Provides training and support to facilitate progress of family and child toward the achievement of specific goals.

OUTREACH:

  • Conducts outreach and delivers presentations to school boards and community partners.
  • Participates in internal and external committees such as Choices for Change and Fetal Alcohol Spectrum Disorders (FASD).
