- Baccalaureate degree in a Health Profession (Nursing or Nutrition)
- Master's degree is an advantage
- Certified diabetes educator (CDE) certification
- Minimum of five years teaching and clinical experience
- Experience in diabetes education and treatment
- Knowledge of health care redesign and quality improvement theory
- Proficiency in using various software and tools
- Strong communication and analytical skills
- Ability to manage projects and collaborate with stakeholders
- Interest in staying updated on diabetes care concepts
- Decisive problem-solving skills
- Valid driver's license and access to a reliable vehicle
- Desire for professional growth and development
- Competency in French is preferred
- Full-time long assignment; 75 hours biweekly
- Approximately 12 months duration
- Includes evening, weekend, and holiday shifts

About the Opportunity
As part of the Primary Health Care – Chronic Disease Management Network, this position oversees committees and leads initiatives to enhance health outcomes across all age groups. Responsibilities include developing practice supports, monitoring trends in diabetes care, and collaborating with internal and external stakeholders. This consultant role does not involve direct patient care but focuses on advancing diabetes care practices in the province.
