Summary of Duties
Reporting to the Manager, Dietary Services, the Dietician is responsible for resident nutrition care programs including promotion of optimal nutritional care; provides nutrition counselling and education to residents and/or their family members and provides nutritional education to Dietary, Nursing and other multi-disciplinary team members as required.
Work Performed
- Identify and recommend policies, procedures and practices that support divisional objectives that contribute to the implementation of strategic initiatives for Dearness Home to meet the Council's Strategic Plan.
- Provide leadership in the execution and delivery of services and programs in the following areas of accountability:
- Complete admission nutritional assessments and determine nutritional risk level upon admission.
- Complete readmission assessments upon resident return from hospital or after significant nutritional changes.
- Complete quarterly and annual nutritional reviews on all residents in the Home.
- Attend all annual care conferences as well as inputting data into the interdisciplinary care conference notes.
- Accurately document fluid intake audits to evaluate hydration status/risk in residents.
- Responsible for completing chart, adaptive aides, and diet order audits twice per year.
- Responsible for completing meal rounds to monitor food and fluid intake of residents and complete chewing/swallowing assessments.
- Complete all referrals within priority sequence, including diet or nourishment changes, seating changes and skin integrity referrals.
- Responsible for developing special menus with resident and family involvement when required and reviewing and signing off on menus and snack cycles twice a year to ensure menus provide optimal nutrition and variety.
- Oversee the nutrition component of the homes enteral feeding program: developing relevant policies and procedures.
- Submit monthly report to the Manager of Dietary or designate.
- Provide inservice education to staff on topics related to nutrition, as required by the facility.
- Process all orders in Synergy on Demand and print as required.
- Participate in the development and evaluation of nutritional policies and procedures.
- Demonstrate commitment to antiracism, antioppression, and human rights through practices and interactions with stakeholders, employees and individuals and support programs and initiatives that reflect this commitment.
- Demonstrate commitment and adherence to Health and Safety legislation and programs; actively promote a culture of safety.
- Engage in opportunities to work across Divisions and Service Areas in support of enterprisewide collaboration.
- Support the development of processes and tools needed to increase efficiencies and improve the effectiveness of services provided by the Unit.
- Provide updates, information, and recommendations to the Manager, Dietary Services on priorities.
- Prepare reports for Council and various Committees as required.
- Gather, assemble, analyze, and present information from various sources to identify, plan and implement actions to achieve various project or divisional objectives.
- Track performance measures for Unit deliverables using appropriate data collection and analytics.
- Effectively and thoroughly evaluate and organize information so that it can be easily understood by various audiences.
- Develop and execute strategies for communicating information and facilitating discussions; Provide rational for decisions, recommendations or courses of action required to resolve an issue.
- Implement strategies to track performance measures for Unit deliverables, using appropriate data collection and analytics.
- Develop and execute detailed project plans to deliver high quality, and effective nutrition programs.
Qualifications/Experience
- Completion of a degree in Food and Nutrition or Food Sciences or Nutrition Science.
- Five years related experience in a Long
- Term Care facility is preferred.
- Registration with the College of Dietitians of Ontario.
- Eligible for membership of Dietitians of Canada.
- Proof of Professional liability insurance.
Skills and abilities in the following areas are necessary:
- Demonstrated knowledge and experience in dietary services with an understanding of the Unit's programs and services.
- Demonstrated knowledge of Ministry of Long-Term Care Program standards and knowledge of Meal Suite software is an asset.
- Demonstrated effective interpersonal and customer services skills, including conflict resolution and problem solving.
- Demonstrated ability to be an effective team member and contribute to programs and services provided by the team.
- Highly effective verbal and written communication skills, with emphasis on presenting findings, preparing briefing material, and providing recommendations.
- Demonstrated ability to effectively communicate with diverse populations within the community; experience working collaboratively
