Job Summary


LAMP Community Health Centre is seeking a Community Health Worker focusing on Preventive Care who will be working on the High Priority Communities Strategy project.

This is a temporary contract position starting in November 2023 and ending March 31, 2024 which is primarily focused on providing upstream and proactive health care for high priority communities through locally informed, culturally responsive interventions to improve access to health and wraparound, social service related supports.

This role will report to the Project Manager, High Priorities Engagement Team Mississauga.


Primary Responsibilities

  • Provide clientcentered screening, consultation and assessment by recognizing the social determinants of health and associated health risks.
  • Develop wholistic health prevention care plans and refer to internal and external service providers to support clients in meeting their health and wellness goals.
  • Lead activities related to the development, implementation and evaluation of health prevention initiatives, policies and procedures, bestpractice and protocols. This includes standardizing the process to create health prevention care plans within LAMP, establishing efficient referral pathways, developing outcome measures, and ongoing evaluation of health prevention initiatives.
  • Actively participate in interdisciplinary team meetings and integrate health promotion into programs and services across LAMP.
  • Work collaboratively with community partners to deliver health prevention programs and related initiatives.
  • Act as a key point of contact for external partners regarding upstream health prevention strategies that address behavioural risk factors contributing to chronic disease.
  • Engage regularly with Ontario Health (OH) and Ministry of Health (MOH) contacts to inform this work
  • Identify and build relationships with various community partners.
  • Provide learning opportunities to stakeholders, students, new hires, and colleagues.
  • Facilitate teamwork and promote group effectiveness.
  • Respect and include people of diverse backgrounds, as well as their perspectives.
  • Build and maintain strong business relationships with partner organizations, participate in outreach to promote LAMP's programs, services and explore partnership opportunities.
  • All other tasks and duties as assigned by the Project Manager.

Required Qualifications

  • 3+ years of relevant experience in community healthcare, health education and program development.
  • Experience working with high priority and atrisk communities, with knowledge of relevant services and community supports in Mississauga and Etobicoke is strongly preferred.
  • Bachelor's degree in health studies, such as health sciences, health administration or kinesiology.
  • Demonstrated problemsolving, organizational, and interpersonal skills with an ability to work effectively and respectfully in a collaborative, interprofessional team environment.
  • Ability to work well under pressure in a fastpaced environment and be adaptable to the changing needs of the organization.
  • Detailoriented with ability to multitask, organize, prioritize, and meet deadlines.
  • Creative and passionate with a desire to learn, rollup their sleeves and lead with an entrepreneurial mindset.
  • Ability to influence positive outcomes and successfully manage conflicts.
  • Strong problemsolving skills and attention to detail and accuracy.
  • Solid working knowledge and ability to navigate a PC windows environment, including shared drives.

Working Conditions

  • The hours may vary (day and evening), with shifts predominantly in the afternoon and evening.
  • Manage clients with complex social and medical needs.
  • May require independent work with variability in interdisciplinary supports.
  • High caseloads must be able to multitask, work in a fast paced, multifaceted environment and make efficient decisions.
  • Travel to multiple locations for clinics, mobile clinics, and outreach. Driver's license (G or G2) and vehicle recommended.
