Admit/Discharge Facilitator - Clinical Neurological Sciences (CNS) Inpatient Unit

The Clinical Neurological Sciences (CNS) program provides patient-centred care to acute neurology and neurosurgical patients.

Specialty care in neurology includes Epilepsy, Movement Disorders, Neuromuscular Disorders, Regional Stroke Program, Regional Multiple Sclerosis Clinic, and Neuro-Critical Care.

Neurosurgery Care includes Neurovascular, Neuro-oncology, Spinal Surgery, Spinal Cord Injury, Head Injury, and Epilepsy Surgery.

The Admit/Discharge Facilitator facilitates and coordinates the movement of patients through the admission, transfer, and discharge processes with a focus on patients with complex discharge needs and participates in decision-making that results in an effective admission and discharge process and the optimal utilization of program and facility resources

Regular Full-Time

Rate of Pay:
$43.04 per hour - $53.80 per hour

Hours of Work: 37.5 hours per week

Qualifications:

Undergraduate Bachelor's Degree in a Health Care Discipline or as accepted by the Hospital
Current registration with applicable college required
Minimum three (3) years recent related experience in a health care environment required
Recent related experience working with CNS patients preferred
Excellent written and verbal communication skills
Proficient knowledge of local and regional health care system
Proficient critical thinking and decisionmaking skills
Proficient people management skills
Ability to build collaborative partnerships
Proficient Microsoft Office skills
Demonstrated practice and commitment to the principles of patient and family centered care
Demonstrated practice and commitment to patient and staff safety at LHSC
Demonstrated practice and commitment to LHSC's Mission, Vision and Values
Demonstrated ability to attend work on a regular basis

Immunization Requirements:

Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox), Hepatitis B, Tetanus/ Diphtheria/Polio; Meningitis.
Provide documentation of the Tuberculosis skin testing (two step) and COVID vaccine series.
