Community Support Worker (CSW)_
WORK ENVIRONMENT
I Have A Chance Support Services Ltd. (IHAC) provides services to individuals who require support in various aspects of their lives.
We are dedicated to help individuals achieve the quality of life they desire by creating a warm positive environment where they can learn, grow and enjoy experiences of what life has to offer.
IHAC strives to support and encourage individuals en route to independence by introducing them to tools and services while in our care.
IHAC offers assistance to people facing a variety of challenges including people with disabilities, mental illness, brain injury, physical disabilities, and medical and forensic concerns.
TASKS
- CSW's are individuals that support and assist clients who are under the care of IHAC.
- Ensure safety of clients, staff and the community by intervening and managing challenging behaviors
- Provide support services to implement all activities as outlined by the Support Home Supervisor or Team Lead
- Provide support services to individuals with respect to maintaining a household, personal health, developing relationships, managing finances, active community inclusion, designated programs, and pursuit of goals identified in the Individualized Care Plan.
- Before start of shift, read all communication notes, memos, and relevant documentation (e.g. incident reports) since last shift worked.
- Complete all logs and designated documentation prior to leaving shift.
- Ensure house and work area is clean for start of next shift and before going out for activities and adhere to the daily responsibilities as outlined for each employee
- Maintain ongoing communication with other team members and maintain positive interactions with team members.
- Administer medications as directed by medical professionals
- Deal with concerns and conflict resolution among team members and clients in an appropriate, professional, and constructive manner.
- Complete regular fire drill, smoke detector, and safety checks of the premises and report findings to Residence Supervisor.
- Maintain a working knowledge of each individual and a professional relationship based on respect and dignity.
- Maintain a working knowledge of all policies and procedures of the agency.
- Maintain your certification as designated. This includes your responsibility to complete any trainings IHAC requires for your employment.
- Continued growth and learning in effort to provide the best possible support.
- Address concerns of individuals and staff to Support Home Supervisor or Team Lead.
- Report situations of suspected abuse immediately to designated personnel for investigation.
- Records for individuals, finances, household, etc. must be promptly and accurately logged.
- Document all concerns and incidents promptly and accurately.
- Matters concerning individuals will be discussed with the individuals, Client Care Manager, and Support Home Supervisor. If questions arise, notify the Support Home Supervisor to answer any questions.
- Know the emergency evacuation plan of the Support Home and protocol for dealing with emergencies.
- Attend regular monthly staff meetings.
- All other duties as required.
EDUCATION/ QUALIFICATIONS
All Community Support Workers must possess, as a minimum:
Post Secondary education in this field working with mental health and addictions.
Vulnerable Sector Criminal Record Check.
EXPERIENCE

8:00 AM - 8:00 AM 2 on 2 off 48-hour shift
8:00 AM - 8:00 PM 2 on 2 off
9:00 PM - 9:00 AM - 2 on 2 off. Awake shift
