JOB TITLE:
Shelter : Prevention, Diversion & Access Support Worker (Internal/External)
DESCRIPTION:
The Shelter Prevention, Diversion & Access Support Worker completes the screening and the intake protocol for individuals and families who are experiencing a housing crisis and provide diversion services and targeted prevention to resolve the immediate housing crisis.

HOURS OF WORK:
Sunday to Thursday, 9:00 a.m. - 5:00 p.m.
QUALIFICATIONS
- Bachelors degree in Social Work, Psychology or other Social Sciences related discipline.
- At least 2 years recent related work experience working with vulnerable populations.
- OR
- An equivalent combination of education, training and experience relevant to the level of work.
- A valid BC driver's license and access to personal vehicle for business related purposes.
- Standard First Aid & CPR.
- Non-Violence Crisis Intervention certificate.
- Harm Reduction and Naloxone certificate.
- Successful clearance of a criminal record and vulnerable sector check
SKILLS AND ABILITIES
- Excellent interpersonal and communication skills in English both written and verbal.
- Strong organizational, advocacy and time management skills.
- Knowledge of theory, principles and practices of Housing First Approach, traumainformed care and harm reduction strategies.
- Full Case management cycle, participant interviewing skills, assessment, care planning, goal attainment, case note documentation, and discharge planning.
- Demonstrated expertise in dealing successfully with persons with mental illness.
- Highly Proficient with a variety of computer software including Microsoft Office suite, Kohezion and HIFIS.
- Strong background and working knowledge of psychosocial rehabilitation processes.
- Demonstrated ability to establish and maintain constructive working relationships with tenants, community groups, government agencies and the public.
KEY DUTIES AND RESPONSIBILITIES
- Responsible for providing support to clients and leading case management planning, activity and reporting for client supports. This requires a sound understanding of case management, navigating complex systems and substance use and mental health conditions.
- Conducts inhome and community visits within Burnaby and surrounding areas.
- Consult, confer, and collaborate with society team members, Social Worker, Nurse Practicioner, Property and Tenancy Team Lead and community resources to achieve excellence in clientcentred care.
- Performs duties such as ensuring adherence to tenancy and program agreements while taking a personcentred approach to support services for participating clients.
- Support clients accessing needed resources (e.g., income supports, legal guidance, tenancy) and pursuing their personal goals (ie. employment, education and training, social and recreational activities).
Salary:
$26.42 per hour
