  • We are hiring a Case Manager on our Gender Affirming Health Clinic.

  • Closing Date: April 30, 2024
  • Contract, 1 year, 35 Hours per Week, Evening Hours and Weekends may be required
  • Salary: 54,800-58,700$ per annum *To commensurate with experience

    Location: There will be a combination of working from home and working in the office/community and directly with clients (Hybrid) depending on program and client needs. The successful candidate will work from the Aurora Office located at 15150 Yonge Street, Suite 201
    Aurora, Ontario, L4G 1M2 on the days of on-site work and Travel for client, with regular travel in York Region.

    Job Profile:

    The Gender Affirming Health Clinic is a service that aims to create a safe, confidential, and inclusive space. We provide culturally competent and gender-affirming care to trans, non-binary and gender diverse people. We work collaboratively with our community partners to ensure supportive and comprehensive care to clients and their families. Our team uses a holistic and compassionate approach when providing services and supports.
    The Case Manager will be part of an interdisciplinary team and will support clients ages 16 years and older with gender identity, transition-related supports and other mental health needs. They will conduct comprehensive intake needs assessments, collaborate with clients in developing treatment plans, and supporting clients to connect with relevant community resources. They will also facilitate groups and support general clinic operations.
    The Case Manager will collaborate with community partners to ensure holistic care for their clients.

    In addition to adhering to all organizational policies and procedures, relevant provincial laws, the regulatory requirements of their profession, Empowerment/Recovery principles, and provincial Intensive Case Management Standards for Mental Health Services and Supports (October, case managers are also expected to behave ethically and demonstrate effective interpersonal communication and teamwork. This involves problem solving, confidence in decision making, relationship building, conflict resolution, and emotional regulation in their work with clients, co-workers, supervisors, and members of the broader community.

    Requirements:

  • Degree or diploma in health or social services plus minimum four years experience or Diploma in health or social services plus minimum one or more years of experience in a community-based organization working with adults with serious mental illness in the community.
  • Full registration and in good standing with the OCSWSSW or a regulatory body under the Regulated Health Professions Act
  • Relevant professional or lived experience with 2SLGBTQ individuals and communities.
  • Strong knowledge and experience of empowerment-oriented case management philosophies and practices Knowledge of systemic issues such as poverty, unemployment, stigma and the isolation felt by individuals with serious mental illness and their families.
  • Second language (Cantonese, Mandarin, Farsi or Tamil, Italian, Korean, Punjabi, Russian, Urdu) reflecting the local community is an asset.
  • Knowledge of the Ontario Mental Health Act, mental health reform principles, the Substitute Decisions Act, the Health Care Consent Act and PHIPPA requirements.
  • Knowledge of PSR and Recovery as guiding principles.
  • Ability to apply Recovery principles and empowerment-oriented philosophies and practices in work with clients.
  • Extensive knowledge of supports and services in York Region and South Simcoe, including formal and informal resources.
  • Knowledge of systemic issues such as poverty, unemployment, stigma, medications and the isolation felt by individuals with serious mental health issues and their families.
  • A valid Canadian Full G driver's license, a minimum $1,000,000 third party liability insurance and proof that insurance covers use of personal vehicle to transport clients
  • A satisfactory Vulnerable Sector Screening (Police Check)
