Description

The Community Health Worker (CHW) is responsible for working within the frameworks of community development (CD) and health promotion (HP) to assist in planning health promotion programs and community initiatives.

They do this by coordinating the implementation of related programing and delivering health promotion and community development programs and services.

The incumbent works closely with other members of Carlington Community Health Centre as well as community partners to develop programming and provide front-line support for these programs.

The CHW facilitates the active participation of community members in issues which have an impact on their health and on the health of the community.


Program-Related Activities

  • Engages in activities with community members to identify community issues and participates in the planning and development of programs.
  • Helps identify programs and resources needed to support activities by developing and maintaining formal and informal networks.
  • Participates in the design, implementation, delivery, and evaluation of programs to meet identified needs.
  • Organizes community activities, facilitates community meetings, administers needs assessments and assists with reporting.
  • Participates in the delivery, evaluation, and maintenance of education programs.
  • Provides information to, and assists, clients in articulating and advocating for health and related services.
  • Participates in outreach strategies, promotes/advertises and informs the community of programs and services.
  • Assists in the recruitment of community members to volunteer time and skills for community development activities
  • Acts as a resource to and advocate for individuals, groups and the community.
