Position Profile:

Our Place Peel is searching for an energetic, enthusiastic and highly motivated Mental Health Coordinator to work with our team of dedicated staff to lead and inspire youth towards positive change.

Using a client-centred approach and the Housing First for Youth (HF4Y) philosophy, the Mental Health Coordinator will support youth by providing case management and supportive counselling to youth in our Emergency Youth Shelters and Outreach Services.

The Mental Health Coordinator will work collaboratively with program staff and youth by providing mental health expertise, guidance, and support through case management to determine appropriate care plans.

The Mental Health Coordinator will assist youth in keeping with the principles of recovery, harm reduction, consumer choice and self-determination.

Role Overview

Responsibilities:

Utilize evidenceinformed assessment tools to determine youth's needs, conduct screenings and coordinate care for appropriate treatment plans.
Advocate for and support youth in navigating and accessing appropriate clinical supports and services related to their wellbeing, mental health and addictions.
Maintain community connections and coordinate with service providers for further referrals to promote a continuum and holistic approach to care.
Maintain and further develop support systems and services within the community to transfer care for optimal sustainable housing.
Utilize the social determinants of health to successfully access and link youth to appropriate supports, community resources, and services.
Identify and assess needs using a proactive and collaborative approach to planned interventions with youth.
Support in the facilitation of a mental health support youth group and collaborate with other staff to support mental health and wellness programming within the shelter setting.
Successfully integrate the philosophy of a strengthbased approach and youthcentered care into daily responsibilities.
Perform administrative tasks as well as other duties as assigned and required.
Manage the input of all information into required data systems.

Qualifications:

Bachelors of Social Work or a Bachelors/Degree in Psychology or equivalent is required.
Post graduate certificate in mental health and addictions is considered an asset.
13 years of experience working with individuals with complex mental health, serious mental illness, and/or substance use.
Youthspecific experience is preferred but not required.
Strong knowledge of recovery principles and models.
Training/experience related to crisis intervention for youth, evidencebased behavioural interventions (CBT/DBT and solutionfocused interventions) and approaches to traumainformed care.
Experience in crisis assessment and highrisk screening.
Knowledge of community resources and the Mental Health Act and privacy legislation(s).
Strong communication and collaboration skills.
Ability to work with diverse communities within an antioppressive and antiracist framework.
Demonstrates diversity and cultural competence; demonstrates understanding of the impact of oppression and social determinants of health.
Demonstrated skills in performing assessments and on the spot risk assessments.
Able to work flexible hours including evenings, weekends, and holidays, if necessary.
Valid CPI/UMAB and First Aid/CPR certifications.
A valid Ontario Driver's License, access to a vehicle and $2,000,000 (minimum) liability car insurance coverage is mandatory.
Satisfactory Police Records Check and Vulnerable position that meets the standards of the agencies policy.
