Description

The Family Support Worker will provide de-escalation and stabilization to families that are in a state of crisis, and which may put the youth at risk of homelessness.

The worker will establish and maintain a respectful, collaborative working relationship with families; providing support, information, modeling, and teaching skills that support effective parenting and healthy family functioning.

These skills include but are not limited to, parent effectiveness, communication, boundaries, behavior management, discipline, child development, relationship building, and effective problem-solving relating to goals.


Case Management Activities:


  • Supports individual youth with establishing and strengthening connections to identified family and natural supports.
  • Supports youth and families through crisis and then develops maintenance plans.
  • Links individual youth and families to appropriate services locally and regionally.
  • Maintains comprehensive knowledge of community services and resources, and provides consultation to community support agencies.
  • Provides case management support and followup for communitybased housing plans and facilitates coordination of services.
  • When necessary, acts as an intermediary between youth and family, and supports by facilitating family mediation sessions.
  • Provides support, crisis intervention, and conflict resolution as required.
  • Advocates on youth's behalf when necessary and act as a positive role model for youth.
  • Liaises with participants, parents, community partners, agencies, donors, coworkers, and volunteers. Develops positive relations with the communities being served by the program and with the general public.

Program Development:


  • With the Manager of the program, supports, creates, and implements approved strategies and plans that will result in the achievement of the program's goals and objectives.
  • Educates staff on resources available to support youth in the community.
  • Liaises with community partners to fortify existing partnerships and ensure warm transfer for youth between programs.
  • Anticipates, identifies, and helps to remove barriers to holistic care.
  • Participates in the planning and development of the Family and Natural Support Program by providing input and working as part of an integrated service model.
  • Establishes and maintains working partnerships with a variety of community agencies, businesses, and members to assist youth in creating sustainable support networks.

Reporting and Other Activities:


  • Completes all mandatory recording and reporting including daily documentation, statistical data collection, and completing registration forms.
  • Provides reports and related information to the Team Leader and/or Manager of Programs as required.
  • Participates in regular supervision meetings and performance appraisal meetings on a semiannual basis.
  • Participates in staff meetings and service delivery meetings as required.
  • Attends training workshops and relevant conferences that pertain to the role.
  • Follows appropriate policies and procedures of 360°kids. Ensures that the program operates in line with 360°kids' mission, vision, and value and that activities comply with applicable standards and laws.
  • Other duties as required.

QUALIFICATIONS OR SKILLS REQUIRED:


  • Post-Secondary degree/diploma in a Child/youthrelated field (e.g. Child and Youth, Social Service Worker, Social Worker, Psychology, Sociology) or the equivalent combination of education and experience working with youth.
  • Minimum of three years of experience working with "atrisk" or streetinvolved youth. Previous experience working with families is an asset.
  • Therapeutic Crisis Intervention training, suicide prevention training, and family mediation training an assets.
  • Current First Aid/CPR certification.
  • Current vulnerable sector screening (criminal background check).
  • Car and valid driver's license with a clean driver's abstract.
