One Full-time Contract to February 2025 (with the possibility of extension or becoming permanent)

The Service Resolution Coordinator will facilitate case resolution for adults 16 yrs and up, who experience high risk situations who have a mental health and/or addictions, or dual diagnosis with a co-morbity of medical situations.

The aim of service resolution is to develop an innovative and collaborative care plan that is co-directed by the leadership of the partnering organizations.
