JOB SUMMARY:

Provide intensive management services for individuals, aged 60 and older, with severe and chronic mental illness. Services are provided in the home, long-term care facilities, or at locations convenient for the client.

Provide proactive outreach to the severely mentally ill, facilitation and support in assisting them to meet their basic needs, and linkage to formal mental health services.


QUALIFICATIONS:


EDUCATION:


  • A diploma or degree in a social or human service related discipline.
  • Demonstrated thorough and comprehensive knowledge of severe mental illness and treatment in the elderly population, and its impact on consumer/survivors and their families, which includes the effects of poverty and marginalization.

KNOWLEDGE/EXPERIENCE:


  • Minimum 23 years experience working with the severely mentally ill elderly population, preferably in a community setting.
  • Personal and/or familial exposure to mental illness would be advantageous.

SKILLS:


  • Skills in counseling, addictions, housing, community outreach, concurrent disorders and general aspects of physical health, are an asset.
  • Must demonstrate excellent communication skills, (both verbal and written), and knowledge of local community resources.
  • Computer skills an asset.

SPECIAL CONDITIONS:


  • Must demonstrate an acceptable comfort level in working with elderly individuals with severe mental illness and be capable of maintaining objectivity in order to perform consistently with the philosophy and objectives of the Geriatric Mental Health Case Management Service.
  • Must possess and maintain a valid driver's license to the class of vehicle operated.
  • Work flexible hours as required in response to community based health hazard.
