POSITION:
ONE (1) TEMPORARY FULL TIME COORDINATOR, SPECIALIZED GERIATRIC SERVICES

The Coordinator is responsible for providing organizational and system leadership for the overall coordination and program development of specialized geriatric services including Seniors' Mental Health, Behavioural Supports Ontario and the emerging Geriatric Medicine Outpatient Clinic.

In conjunction with the Manager, the Coordinator works collaboratively with North East partners in system level planning for the provision of quality, evidence informed care to older adults.


QUALIFICATIONS

  • Minimum four (4) year Bachelor's Degree with a specialization within a health related field from an accredited university
  • Current certificate of registration in good standing with an applicable regulatory college is required.
  • Minimum of five (5) years' experience working as a health professional in a health care environment with a focus on older adults
  • Minimum of two (2) years working in a leadership/supervisory role, preferably in a health care environment
  • Demonstrated knowledge of best practices for dementia, delirium, depression and other mental health issues and their effect on patients, families and service providers.
  • Demonstrated knowledge in the management of geriatric syndromes, including delirium, polypharmacy, falls and functional decline, acute/subacute cognitive changes, bowel/bladder, and nutrition/hydration problems.
  • Demonstrated knowledge of Specialized Geriatric Services across the care continuum including primary care and community support services (i.e. Alzheimer's society, Home and Community care).

  • Demonstrated excellence in clinicalreasoning, critical thinking skills, assessment, documentation, intervention and care planning skills
  • Demonstrated ability to interact with physicians and other healthcare providers in a collaborative, professional, intelligent, and effective manner.
  • Strong leadership, communication, team building and creative problem solving skills
  • Demonstrated cultural awareness and competencies in working with patients and care partners from diverse backgrounds.
  • Experience with change management, quality improvement and innovation
  • Proficient computer skills particularly with Microsoft Office (Word, Excel, Outlook) and patient information systems.
  • Understanding of multiple regulated health professions and associated Acts
  • Familiarity with collective agreements, scheduling
  • Demonstrated time management, prioritization and organizational skills
  • Demonstrated commitment to a culture of safety and prevention of adverse health events required
  • Must demonstrate good attendance
  • Bilingualism may be required as determined by compliance with the French Language Services Act

North Bay Regional Health Centre (NBRHC) is a unique health care organization with three primary roles: providing acute care services to North Bay and its surrounding communities, functioning as the district referral centre providing specialist services for smaller communities in the area, and it's the specialized mental health service provider serving all of northeast Ontario.
