Case Manager
Status:
Full Time Permanent (Hybrid: remote with some onsite work)
Working Hours:37.5 hours/week, Monday to Friday 8:30am to 4:30pm, plus on-call rotation
Reporting to:
Director, Residential Care & Case Management
Job Summary:
The Case Manager assesses client/caregiver needs, determines eligibility for community services, manages program waitlists, and develops plans based on the client/caregiver needs and preferences.
Supports clients/caregivers by facilitating appropriate referrals, crisis management, and supporting the delivery of client/caregiver events and programs. Provides guidance and leadership to staff. Represents the agency at community tables and networks. Supports a culture of client safety as part of ESS key strategic goal and priority.Identifies client safety risk; supports a non-punitive reporting environment for reporting unsafe practices and/or errors; supports staff safety surveys and acts on the safety recommendations.
Duties and Responsibilities:
- Assesses and identifies the individual needs and risks of clients/caregivers and determines eligibility for community support services.
- Manages a diverse mix of caseloads, establishes goals, and develops clientcentred plans and recommendations in collaboration with client's circle of care.
- Organizes and schedules appropriate supportive services for clients/caregivers and facilitates appropriate referrals to internal and external programs.
- Manages program waitlists, enters, and maintains accurate program data.
- Facilitates and supports client/caregiver events and programs.
- Accesses and maintains uptodate and confidential client case records.
- Performs regular reassessments in accordance with agency policy and provides education, advocacy, and crisis management to clients/caregivers.
- Gathers and analyses information relevant to clients and programs and provides reports on program progress.
- Maintains professional networks with community partners to connect clients with community resources and to address the needs of the senior population.
- Coaches, mentors, and provides leadership and guidance to staff in relation to client care concerns as needed and appropriate.
- Performs other duties as required.
Qualifications:
- Post secondary degree or diploma in social work, social services, or related field.
- Minimum 13 years clinical experience in community care, home care, case management, crisis intervention, and program/service facilitation and delivery is preferred.
- Experience working with the elderly and individuals living with dementia.
- Strong knowledge of caregiver issues, the health care system, and community resources.
- Knowledge and training of InterRAI CHA assessment tool is an asset.
- Exceptional organization & time management skills to manage multiple and conflicting priorities and to minimize risk.
- Excellent communication (verbal & written) and interpersonal skills, high level of initiative.
- Able to work well independently and within a dynamic team.
- Must be willing to travel. Valid Ontario Driver's License and use of a reliable vehicle is required.
- Valid satisfactory Vulnerable Sector Screening Check.
- Able to provide proof of full COVID19 vaccination upon hire.

$49,666.50 per year
