Position Summary:


To provide case management services for a caseload of clients with severe mental illness who are homeless, at risk of homelessness or transitioning to independent living in the Mississauga/South Etobicoke area.

Case Managers work in partnership with clients to develop the skills and supports they need to live successfully in the community.

Clients are supported through visits in their own home, in the community or in the office.


Responsibilities:


  • Manage a full caseload, providing case management support to assigned individuals
  • Complete assessment reports outlining individual psychosocial needs and determining eligibility for the program
  • Engage individuals in the Recovery process through a goal oriented, skillsbased approach
  • Ongoing monitoring of individual's wellness, assessment of risk and overall progress in one's Recovery Process
  • Liaise with existing treatment team including physicians

Key Qualifications:


  • Bachelor's or Community College degree in a healthrelated discipline
  • Current registration with a recognized college preferred
  • Over 5 years' experience working with adult clients diagnosed with serious mental illnesses
  • Experience working with people who are homeless or have frequent contact with shelters an asset
  • Experience working with clients with substance misuse issues is an asset
  • Working knowledge of community resources
  • Training and working knowledge in the area of Recovery
  • Proficient computer skills
  • Excellent communication, organization, presentation and problemsolving skills
  • Treatment planning and Client intervention experience required
  • Valid driver's license, relevant insurance and use of own vehicle
  • Demonstrated ability to work independently and as a part of a team
  • Ability to speak a second language would be a definite asset
  • Proven skills in the area of Crisis Intervention and Suicide Risk Assessment
