- Be accountable for assessing and treating assigned outpatients to enhance their safe and independent functioning.
- Offer guidance, supervision, and education to rehab assistants, OT student placements, volunteers, clients, families, and community members as required.
- Maintain accurate records of assessments, treatment plans, progress notes, patient attendance, and other relevant data following departmental protocols and regulations.
- Collaborate with medical staff, other healthcare professionals, and community agencies involved in patient care.
- Ensure compliance with occupational health and safety regulations to maintain a safe work environment.
- Report any unusual incidents promptly to the manager.
- Work specifically in the Outpatient program (Neuro and Upper Extremity).
- Adhere to the professional guidelines set by the provincial college of Occupational Therapists.
- Participate in patient safety programs, identifying and reporting adverse events/near misses.
- Attend safety training and educational sessions and address any unsafe conditions or practices promptly.
- Engage in team conferences, presentations, educational seminars, and relevant committees.
- Promote a culture of safety for clients and staff by following safety protocols set by SJHC.
- Foster an environment that supports positive change utilizing change leadership principles.
- Enhance sensory-perceptual motor function and psychosocial skills.
- Promote independence through treatment and adaptive device utilization.
- Improve physical and mental function in self-care and life skills.
- Evaluate and provide appropriate wheelchairs, adaptations, and training for patients.
- Select, fabricate, and fit adaptive devices to enhance ADL performance.
- Assess and train patients in homemaking skills and participation in group activities.
- Plan patient discharge in coordination with the team.
- Assess life skills performance and address architectural barriers in the home environment.
- Recommend home environment modifications to enhance independent living.
- Advocate for follow-up programs for patients.
- Master's in Occupational Therapy or B.Sc. degree in Occupational Therapy or equivalent. Must be registered with the College of Occupational Therapists of Ontario.
- Membership in the Canadian Occupational Therapy Association is preferred.
- Excellent time management, organizational, and prioritization skills related to patient care.
- Strong communication and clinical reasoning abilities.
- Customer service-oriented with the ability to work independently and seek guidance when needed.

The Job Summary:
The Outpatient Occupational Therapist will deliver patient care by conducting assessments, developing personalized treatment plans/consultations, administering direct or indirect therapeutic interventions, assessing treatment progress, discontinuing treatment when appropriate, and referring patients to other resources/services as needed. Reporting to the Manager of Ambulatory Care and Outpatient Therapy Services, the therapist will be accountable for delivering high-quality care and upholding departmental standards.
