The Toronto Grace Health Centre (TGHC) provides specialized care and services to those individuals who require Complex Continuing Care, Post-Acute Care Rehabilitation, Palliative Care, and Transitional Care.

The care and services provided at TGHC help facilitate the patient flow through the health care system and ensures recovery to those individuals who may return to their homes or to Long Term Care (LTC) facilities in their communities.

TGHC operates in 3 sites - the Main site (Church& Bloor St), Harbour Lights (Queen & Jarvis Street) houses and a Specialized Care Centre (Finch& McNicoll)

***Reporting to the Director of Clinical and Transitional Care Programs, the Patient Care Manager (PCM) will provide administrative and clinical leadership in the delivery of high quality patient-centred care which is consistent with the philosophy policies and guidelines of Toronto Grace Health Centre.

CCC3 is the home of the Grace Recover for Chronic Critical Illness LTV unit and Respiratory CCC. 30 are dedicated for ventilated patients. The patient population includes multi-factorial diseases and injuries with a unique interdisciplinary care approach.

The mission of the Grace Recover Program is to support patients with ventilator liberation and increase functional capacity to support eventual community discharge.

We also enable patients to transition into our home ventilation program in partnership with TGHC's Remote Care Monitoring Program. The unit is staffed with critical care physicians, respiratory therapists and a mix of RN & RPN designations.

The care model is primary with a hybrid modular assignment system to support continuity of care and levels of autonomy.

The unit also utilizes modern technologies and safety systems to support flow, utilization and optimal patient care.

The Patient Care Manager utilizes the principles of management and evidence based practices and processes to achieve optimal results. The Patient Care Manager works collaboratively with all stakeholders and is part of the Management Team.

This role is an exciting opportunity for a highly motivated individual who excels in working with an interdisciplinary team and is able to develop, implement and evaluate initiatives that advance the strategic directions of the hospital.


KEY RESPONSIBILTIES
The PCM is a leader, coach and mentor for nurses, allied health professionals and administrative staff on the unit. The PCM works collaboratively with stakeholders in the development of goals and objectives for the patient care unit.

The PCM is accountable for fiscal planning; provide leadership for operating initiatives to meet strategic directions; and work in collaboration with the Interdisciplinary team members and the Medical Director of the Complex Continuing Care Program.


QUALIFICATIONS

Education and Qualifications:


  • Bachelor's Degree in Nursing; Master's degree is preferred
  • Current Certificate of Competence issued by the College of Nurses' of Ontario in good standing
  • Minimum of 3 to 5 years working in a hospital; minimum 2 years of experience leading an Interprofessional health care team
  • Experience in Critical Care preferred
  • Experience with community based programs and services preferred
  • Selfdirected; ability to handle a wide variety of administrative tasks
  • Wellorganized with an ability to multitask, prioritize and meet deadlines
  • Superior interpersonal skills and high degree of emotional intelligence
  • Proven change management skills combined with the ability to work with others
  • Proven ability to promote an academic practice environment within the clinical setting
  • High degree of professionalism, integrity, maturity, discretion, initiative and judgment
  • Selfmotivated, team player, and ability to thrive in a fast paced environment.
  • Excellent oral and written communication skills
  • Demonstrated proficiency in Microsoft Office; advanced knowledge of Word, PowerPoint, Outlook, Excel)
