Building a community of hope since 1987, we would like to invite you to join our journey. Siloam Mission is currently seeking for a Permanent/Full-Time Youth Case Worker.


POSITION DUTIES

  • Prioritize housing locations with participants, assisting them in finding permanent housing as quickly as possible;
  • Develop individualized service plan in collaboration with participant addressing short term and longterm goals;
  • Assist participants in all areas of life in order to transition into interdependent living ie. life skills, budgeting, cooking, hygiene, healthy relationships etc.
  • Assess participants relevant skill strengths and development needs;
  • Connect participants with appropriate community services by assessing their needs and acting as a liaison between participants and agencies;
  • Provide strengthsbased case management and service coordination services designed to assist participants to obtain and maintain housing stability;
  • Establish partnerships with landlords, and advocate on behalf of program participants;
  • Meet with participants in the community and in their homes to assess progress and ensure service plan outcomes are met or changed as needed;
  • Act as an advocate for the needs of the participants of Siloam Mission;
  • Maintain participant records in a timely manner;
  • Monitor, evaluate and communicate progress of participants with the participants themselves, team members and the supervisors on a regular basis;
  • Participate in Naatamooskakowin (Coordinated Access) by facilitating and documenting Sharing Experiences and attending Community of Practice meetings

ESSENTIAL

  • A degree or diploma from a social science field;
  • Experience in the Child and Family Services field working with the youth or young adults;
  • Knowledge of child welfare and it's governing bodies;
  • Strong understanding Indigenous Cultures, and its relation to child welfare;
  • At a minimum, a strong comfort level of Indigenous Spirituality and its relationship to traditional Christian beliefs;
  • Demonstrated ability to work independently as well as in a team;
  • Ability to work from a trauma informed perspective;
  • Experience in mental health and/or addictions;
  • Experience in case management and goalsetting;
  • Excellent interpersonal skills and the ability to form relationships with marginalized individuals;
  • Knowledge of community resources for homelessness, health, mental health and substance use;
  • Demonstrated understanding of the unique issues and barriers affecting the participants;
  • Highly developed understanding of and proven ability to communicate appropriate professional boundaries;
  • Experience working in Housing First and an understanding of a variety of housing models including Housing First and Recoverybased preferred
  • Experience in responding appropriately to potentially violent situations using NVCI;
  • Demonstrated knowledge of NVCI, ASIST, CPR/First Aid, Mental Health First Aid training (MHFA), Motivational Interviewing is preferred
  • Must have a valid class 5F driver's license, a vehicle in good working order and be willing to drive participants to appointments;
  • Ability to work with a diverse staff team and promote inclusion and cultural awareness in the workplace;
  • Comfort in reaching out to youth and young adults at risk of or experiencing homelessness and building professional rapport
  • Demonstrated understanding of the unique issues and barriers affecting those experiencing poverty and homelessness;
  • Ability to work within a harm reduction framework;
  • Knowledge of Housing First Principles and housing resources;
Excellent critical thinking and problem-solving skills;
Excellent written and oral communication skills;
Ability to communicate appropriate professional boundaries
MS Office and data entry skills;
Experience in working with HIFIS4 an asset


PREFERRED
Clear Vulnerable Sector Check and Clear Adult Abuse Registry Check will be required.
