The Consultation Liaison Team at THP provides expert service and psychiatric consultation for individuals who develop signs and symptoms of psychiatric distress while they are in hospital for a medical or surgical reason.

Our team will liaise with the treatment teams to optimize the bio-psycho-social care of patients.


As an integral member of the multi-disciplinary team, the Addictions Case Manager will work collaboratively with the interdisciplinary team to provide service for patients admitted to medical/ surgical units across the hospital experiencing mental health and substance use issues.

The Addictions Case Manager will utilize a person-centered approach that is grounded in harm reduction and recovery principles to identify, screen, assess, consult, and coordinate care for identified individuals.


Key responsibilities include:

  • Assess the level, pattern, and dynamics of substance use by an individual, as well as the client's overall functioning, including physical and mental health, and their social support network dynamics.
  • Complete comprehensive risk assessments (selfharm, suicidality, harm to others, safety etc).
  • Provide education, support and resources to individuals, families, and healthcare providers to reduce harm and risk related to substance use.
  • Promote positive choices through empowering individuals to become an active part of their own recovery journey.
  • Collaborates with the patient to develop a discharge plan by supporting them in the development of a recovery plan, and connection to resources and community services.
  • Ensures coordinated delivery of care through ongoing communication with all members of the treatment team.
  • Establishes relationships with other community groups and relevant services that will support the client's functioning.
  • Ensures continuity of service to patients through a continuous process of service matching, coordination, and outreach to resource.
  • Maintains patient records including but not limited to assessment findings, progress notes and discharge summaries.
  • Participate in program quality initiatives, and program evaluation as necessary.

Key Qualifications:


  • Professional qualification in a health discipline with addictions specific training required.
  • Certification through the Canadian Addiction Counsellors Certification Federation considered an asset.
  • Several years' experience working with adult clients with mental illness and substance use issues.
  • Demonstrated ability to assess client case management needs, implement a service plan and facilitate referral to treatment programs within a short term model of care.
  • Experience with utilizing motivational interviewing skills and techniques.
  • Knowledge of community resources.
  • Excellent communication, organization, presentation, and problemsolving skills.
  • Demonstrated ability to work independently and cooperatively with community partners.
  • Ability to speak a second language is considered an asset.
  • Proficient computer skills.
  • Reliable, flexible, and collaborative team player.
  • Positive work and attendance record essential.
