JOB DESCRIPTION


Under the direction of the Director of Care (DOC) or Assistant Director of Care (ADOC), the RAI Coordinator, shall be responsible for providing education, monitoring and on-site support to all Resident Assessment Instrument (RAI) users in the Home.


This role supports the sustainability of the RAI-MDS clinical tool requirement and provides training in its use for care planning and task implementation.

The RAI Coordinator will coordinate the accurate and timely completion and submission of RAI-MDS data to management, the Canadian Institute for Health Information (CIHI) and/ or Ministry of Health and Long-Term Care (MOHLTC) or other regulatory bodies.


RESPONSIBILITIES

  • Coordinate and monitor RAI-MDS processes according to home policies, MOHLTC regulations, College of Nurses Standards of Practice, and interRAI requirements (CIHI).
  • Collaborate with RAI users to develop efficient processes to collect data, and complete assessments that ensure accurate and individualized care plans development as well as coordinate and support annual RAI-MDS proficiency training with all RAI-MDS users.
  • Ensure RAI-MDS assessments, resident assessments protocols and care planning are completed within required timeframes.
  • Support, implement and evaluate continuous quality improvement tools and forms to collect data from various nursing programs and analyze outcome scores.
  • Ensure that RAI-MDS 2.0 quality indicators are validated and accurate.
  • Generate RAI-MDS 2.0 reports and facilitate their use in clinical, and departmental operational decision making for quality management purposes.
  • Review/audit all Resident Utilization Groups for accuracy and maximum potential prior to data submissions to the CIHI, in collaboration with the DOC and/or delegate.
  • Submit RAI-MDS assessments to CIHI within required timeframes and review CIHI reports and complete resubmissions when necessary.
  • Liaise with electronic health record software vendor when necessary.
  • Complete all nursing requirement processes for new resident admissions.
  • Assist in supervising the nursing care provided by the RPNs and PSWs, and if applicable, RN staff members.
  • Participate in, and demonstrate an understanding of, resident safety principles and practices in all day today activities, following all safe work practices and procedures and immediately communicating any activity or action which may constitute a risk to resident safety.
  • Work in compliance with the Occupational Health & Safety Act and the Long-Term Care Homes Act, and their regulations in performing duties in a safe manner and follow all relevant policies, procedures, and legislation.
  • Maintain confidentiality in accordance with all applicable privacy and protection legislation.
  • Perform any other relevant duties necessary for the welfare of the residents and efficient operation of the Home.

QUALIFICATIONS

  • Current member in good standing with the College of Nurses of Ontario
  • Current RAI-MDS certification as per Ministry of Health and Long-Term Care requirements
  • Minimum two (2) years Longterm Care (LTC) clinical, project experience including experience with resident admissions
  • Sound knowledge of the LTC Act and MOHLTC Program Standards, including documentation requirements
  • Knowledge and experience with adult education strategies/ facilitation/ presenting to groups
  • Ability to generate and understand RAI-MDS and CIHI Quality Indicators reports and make recommendations on same
  • Experience in the use of the Point Click Care system
  • Proven problemsolving, assessment and analytical abilities
  • Well organized, with excellent time management skills and a high level of technological proficiency
  • Demonstrated success leading new initiatives
  • Welldeveloped oral and written communication skills
