Detailed Overview:
Reporting clinically to the Professional Practice Chief, Social Work and administratively to the Manager, Clinical Operations, provides comprehensive social worker services and therapeutic clinical counseling to patients and families for an the assigned clinical specialty area; conducts comprehensive social work and psychosocial assessments of patients and families using a variety of assessment tools and in accordance with professional standards, clinical policies and practices; educates and provides consultation to patient, families and other healthcare professionals by providing leadership in patient care planning for the effective utilization of patient resources; participates as a member of local committees and planning bodies to identify areas for improved efficiency and effectiveness in service delivery to enhance patient care services and reduce gaps in the service; develops, coordinates and delivers in-services, presentations, psycho-educational and support groups and initiates, develops and carries out research activities


Responsibilities:

  • Conducts comprehensive social work and psychosocial assessments of patients and families by interviewing the patient and/or family, obtaining relevant information, gathering social data regarding the patient and/or family and interpreting the data using a variety of tools, in accordance with professional standards, clinical policies and practices.
  • Provides comprehensive therapeutic clinical services such as grief and bereavement counseling to patients and families in the assigned clinical specialty area by exploring the emotional psychosocial response to identify problems, personal concerns, treatment objectives and facilitates change in coping style, behavior, attitude and feelings as it relates to the illness and other system issues; participates with patient, families, community supports and the interdisciplinary team to coordinate services, manages cases and facilitates meetings.
  • Provides consultation and education to patients, families and other healthcare professionals on issues related to high risk factors, recovery and current practice in the field of adaptation such as illness and disability, trauma and bereavement, critical incident stress management, protection of vulnerable adults, chemical dependency, community living and support services.
  • Maintains patient records by documenting patient assessments, charting information, preparing progress notes and reports in accordance with established standards, policies and procedures to comply with regulatory requirements; documents patient information for future reference.
