Psychogeriatric Case Manager.

JOB SUMMARY:

Note: _Staff will be required to work in residents' rooms/apartment units and may therefore be exposed to secondhand smoke._ _Occasional evening and weekend hours may be required. Frequent travel will be required using own vehicle._


WHAT YOU WILL DO:


  • Assists clients in the development and implementation of a Behavioural Support Plan tailored to the client's needs and issues designed to improve quality of life and assist in the mitigation of responsive behaviours through intense behavioural supports that can include increase in frequency and duration of inhome visits. Identify and refer to resources specific to those needs and issues to ensure sustainability and successfully remaining in their own homes;
  • Complete required assessments in order to produce a comprehensive PIECES/Functional Behavioural Assessment;
  • Complete PIECES/Functional Assessment and provide timely and accurate record keeping and reports as required;
  • Provide support, ongoing training and consultation to colleagues, family members and/or other caregivers to assist in mitigating escalating responsive behaviours;
  • Provide supportive counselling or referral and linkages to health care, programs or service providers;
  • Provide crisis management/intervention and assist in the development of crisis plans for clients;
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles;
  • Maintain strict confidentiality and follow all other required policies and procedures;
  • Consistently interact with clients, staff, family members, other community agencies and partners in a professional manner that reflects favourably on the organization and the Behavioural Supports Ontario Project;
  • Effectively participate as a member of the Mobile Support Team; attend staff/team meetings, client care conferences, other multidisciplinary meetings, committee meetings etc. as required;
  • Attend and actively participate in all required training;
  • Liaise, collaborate and coordinate with other professionals, service providers and partners within the Behavioural Support Services;\
  • Complete other duties, from time to time, as assigned by the Coordinator and/or Program Director.

WHAT YOU BRING:


  • Completion of a diploma/degree in Social Services/Human Services or equivalent;
  • Minimum three years related experience in providing support to vulnerable seniors and older adults with responsive behaviours related to mental illness, addictions, dementia or other neurological conditions;
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with clients on goals/priorities;
  • Crisis intervention skills, with welldeveloped problem solving abilities and interpersonal skills;
  • Training/knowledge of P.I.E.C.E.S., U-First approaches or willingness to receive training;
  • Experience/familiarity with RAI documentation, and OCAN and health profession standards for documentation of client information;
  • Sound understanding and commitment to the principles of the Behavioural Supports Ontario (BSO) project and the Action Plan for the Central LHIN;
  • Ability to collaborate effectively with other members of interprofessional care teams, clients, family members and community team members;
  • Good oral and written communication skills and computer literacy, including MS Word and Excel;
  • Counselling and responding skills;
  • Creativity and flexibility to approaches;
  • Knowledge of community resources and supports;
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies;
  • Strong assessment skills;
  • Understanding of cultural competency and framework
  • Valid drivers' license and access to own vehicle is required;
