Purpose of Position
The Rights of Passage Team Leader provides leadership to the program and staff.

Team Leaders oversee case management plans for program participants, supervise program Youth Workers, and manage day-to-day operations of the program.

Responsibilities:

Staff Supervision

Provides direct supervision and coaching to five (5) Full Time Youth Workers and approximately six (6) per diem Youth Workers. This includes ongoing coaching, facilitating regular supervision meetings and completing Staff Annual Performance Appraisals
Interviews Youth Workers when there are vacancies
Responsible for the direction and management of the daily shift including organizing scheduled activities and ad hoc activities at ROP
Responsible for ensuring that adequate staffing is in place for all shifts

Case Management

Supports Consistent Workers in developing case management plans for participants
Facilitates monthly Case Management Team meetings and regular case review
Ensure that the case plans developed are consistent with the policies and procedures of CHT
Liaise, when necessary, with community agencies to gather information and coordinate case plans for participants
Consult with Youth Workers regarding referrals to internal and community services
Ensure case management activities are completed (Assessments, reviews, communication and documentation)
Safeguard confidentiality, professional ethics, and standards throughout the case management process

Counselling

Meet with participants for regular check in on their progress
Provides individual counseling support to participants, and addresses their issues
Provides participants with referrals to community services that they need
Assists participants in interfacing with outside systems (welfare, subsidized housing etc.)
Facilitates Life Skills groups with participants

Documentation

Complete ETO touchpoints and ensure Youth Workers are completing their touchpoints on time and accurately
Oversees case plan direction activity and progress
Oversees clear communication of case review decisions to all relevant parties
Summarizes case plans and share them with community agencies on behalf of Participants when needed

Administration

Supports program outreach and intake processes
Leads / attends case management team meetings
Attends daily shift change meetings and monthly team building meetings

Data Quality Assurance

Monitors ETO and paper files to ensure quality assurance standards are met
Delivers regular, direct supervision to Youth Workers in accordance with the HR policies and procedures
Advocates with community agencies on behalf of the participants
Reconciles participant savings, petty cash and tokens with the accounting department
Participates in specific agency projects or community initiative as required
Establishes and maintains positive relationships with participants of McGill/Granby Village, police, fire departments and community agencies

Qualifications:

Bachelor of Arts, Bachelor of Social Work, and a minimum of 2 years' experience in a residential setting or CYW and a minimum of 8 years relevant experience in a residential setting
Knowledge of adolescent developmental theory and attachment theory
Experience and skills in traumainformed approach and strong harm reduction skills
Possesses a high level of cultural competency to the role
Leadership skills
Knowledge of community resources
Excellent written and verbal communications
Familiarity with the delivery of Life Skills
A minimum of 2 years' experience working with young people experiencing homelessness and/or survivors of human trafficking in a residential setting, administration, and employee supervision
Previous experience working with staff from diverse backgrounds, including racialized and 2SLGBTQ+ staff is an asset
