Position Description:

Temporary assignment - approximately 52 weeks

***: The Youth Wellness Centre (YWC) provides care to youth aged 17-25 who are experiencing significant mental health and addictions concerns. The YWC Social Worker provides quality social work services congruent with the Code of Ethics and Standards of Practice of the Ontario College of Social Workers and Social Service Workers (OCSWSSW). The Registered Social Worker provides psychosocial assessment, counselling/psychotherapy, case management, youth/family education, supportive counselling, transition planning, service coordination and consultation according to best practice standards in order to assist youth and their families to achieve optimum recovery and quality of life. This includes maximizing the benefit youth and families receive from their medical treatments and transitioning to risk-reduced, timely discharges. The Registered Social Worker provides leadership in high-risk social situations, program development, advocacy, and developing and maintaining strong community linkages. The Registered Social Worker will also participate in mentoring, community development, teaching, continuous quality improvement and research activities.


QUALIFICATIONS:


  • Bachelor of Social Work Degree (MSW preferred) in Clinical Social Work
  • Registration in good standing with the Ontario College of Social Workers and Social Services Workers
  • Membership with the Ontario Association of Social Workers preferred
  • Minimum two years experience in a comparable role working with the defined population (eg youth with complex mental illness concerns and their families)
  • Knowledge and current communitybased experience working with youth with mental health and addictions concerns and their families required
  • Current and demonstrated knowledge of the unique developmental issues of young adult clients and their families required
  • Current and demonstrated knowledge of the early stages of mental illness and addictions including etiology, prognosis and treatment modalities required
  • Maintain current working knowledge of relevant legislation which includes the following: Mental Health Act; Health Care Consent Act; Substitute Decisions Act; Public Hospitals Act; Long Term Care Act; Child and Family Services Act; PHIPA and Excellent Care for All Act
  • Understanding the use of psychotropic medication in youth, psychiatric diagnosis and formulation of a mental health assessment
  • Ability to work in a collaborative partnership with clients, family, and community
  • Communication skills, interviewing skills, critical thinking and problemsolving skills
  • Participation in research, best practice, program evaluation, and evidence of continued professional development and education
  • Computer skills in Microsoft outlook, windows, word, excel, and Powerpoint
  • Ability to work under pressure, prioritize workload and meet deadlines
