Description

The Transitions Team at Holland Bloorview supports the transition of youth with disabilities and their families to the adult healthcare system and a meaningful adult life. Interventions for clients and families are provided in a variety of innovative formats including individual solution-focused coaching consultations, groups, large events, and partnership programs with community agencies. The Transitions Team is also involved in systems-level changes to bridge pediatric and adult services, and promote joint accountability in meeting client and family needs.

Key Responsibilities

The Life Skills Assistant works closely with the Social Worker (SW) and Occupational Therapist (OT) to support implementation of assigned interventions related to the transition to adulthood:

  • Facilitating small group intervention, in-person or virtually, and education sessions under guidance of SW and/or OT
  • Assistance with running large transitiontoadulthood events
  • Individual sessions with clients and/ or their families for resource sharing, inperson or virtually
  • Involvement in community partnership and capacity building initiatives with adult health and social care agencies in collaboration with the SW and/or OT

In addition, Life Skills Assistant will:

  • Complete regular client documentation as outlined
  • Assist with event scheduling, coordination and materials setup
  • Communicate and meet regularly with overseeing SW and/or OT
  • Complete project work to support the development of innovative transition interventions
  • Act as a representative of Holland Bloorview with adult health and social care agencies in the community by maintaining open, professional communication and a collaborative working relationship
  • Adhere to and ensures the maintenance of the applicable Standards of Practice
  • Remains current with professional literature related to best practice evidence for the transition to adulthood for youth with disabilities
  • Participate in quality improvement initiatives, support the maintenance of a safe and healthy work environment and advance a culture of client/patient safety through work and daily practices

Qualifications

  • Diploma in Therapeutic Recreation from a recognized Canadian College (R-TRO DIP designation), postsecondary diploma in recreation & leisure, certificate from an accredited Occupational Therapist Assistant Program, and/or a diploma from an accredited Social Service Worker Program
  • Experience working with youth with physical and developmental disabilities and their families
  • Must be willing and able to travel to a variety community locations across the GTA
  • Must be willing to work hours according to varying requirements of a participant interventions including evenings and weekends


  • Basic CPR certification

  • Basic Rescuer (C) certificate an asset
  • Non-Violent Crisis Intervention (NVCI) Certificate an asset
  • Experience in individual and group interventions, and project work an asset
  • Demonstrates ability to problem solve, be flexible, and to assess the needs of individuals and to respond quickly to needs
  • Demonstrates initiative and selfdirection to work independently at times
  • Demonstrates ability to organize time and manage working hours
  • Demonstrates ability to work in collaboration with staff/clients/families and community partners
  • Demonstrates sound judgment and excellent communication skills
