Central West Specialized Developmental Services (CWSDS) is a leader in providing supportive living, community participation supports and specialized services, assessment and treatment to people who have developmental and intellectual disabilities, dual diagnosis, and/or challenging behaviours throughout the municipalities of Halton, Peel and Dufferin and the regions of Waterloo and Wellington.


As a specialist in the field of developmental and intellectual disabilities, CWSDS has experienced continued growth over the last five years.

As such, we are looking to hire a
Resource Professional, Dual Diagnosis Servivces, on a full time contract basis, for approximately 18 months (maternity leave contract).

This position will report to the Manager, Halton Support Services and Dual Diagnosis Services.


RESPONSIBILITIES:


RESPONSIBILITIES:


  • Working with individuals, caregivers and Dual Diagnosis Service (DDS) team members to develop the persons and caregivers' treatment goals and objectives, within the geographic area of responsibility (Halton, Peel and Waterloo regions, Dufferin and Wellington counties).
  • Working closely with all disciplines participating in intake, assessments/screening tools, information gathering, environmental scans and data collection.
  • Coordinating community resources in support of person's needs and assisting the person in utilizing these resources.
  • Liaising with person and their families to assist and support them.
  • Assist in monitoring response to treatment recommendations.
  • Participating in a multidisciplinary team to develop treatment plans for assigned person.
  • Conducting observations in all environments (i.e., home, school, day program etc.) to provide guidance/information to person, families and community partners.
  • Providing education and training to caregivers and community providers on a variety of topics (i.e. Dual Diagnosis, behavioural strategies etc.).

QUALIFICATIONS:


  • Developmental Service Worker Diploma or Degree/Diploma from a recognized Community College/University in Social/Human Services (minimum 2year program)
  • Significant experience in delivering social support services to individuals with developmental disabilities and/or mental illness.
  • Previous service coordination and case management experience.
  • Good knowledge of the social services resources and the professional service providers within the greater community area.
  • Ability to coordinate, facilitate and provide training to community service providers.
  • Effective assessment, planning, priority setting and problem solving skills.
  • Good oral/written communication skills.
  • Ability and willingness to work flexible hours to meet with the individuals and their community supports.
  • Valid Ontario G class driver's license and a vehicle to conduct home visits and community outreach activities.
  • Providing proof of vaccination for COVID19 and maintaining vaccination status as necessary from time to time during employment.


The successful applicant will be hired in accordance with the provisions set out in Article 10.04 of the current Collective Agreement.


Hours of work:
Monday to Friday, 8:30 am to 4:30 pm. Flexibility is required at times.


Location: 53 Bond St, Oakville, L6L 1K8. At the present time, this position is working on a hybrid model (home and office, 2-3 days/ week split). This may change as per organizational needs. Willingness to drive within the central west region (Halton, Peel, Waterloo, Guelph, Dufferin, Wellington).
