Key Areas of Responsibility

  • Arranges for the admission, discharge or transfer of residents in the Long-Term Care Home and coordinates the LTC orientation.
  • Carries out all Long-Term Care admissions, discharges and transfers while complying with the policies and procedures of the Toronto Central LHIN, Toronto Community Care Access Center and the Ministry of Health and Long Term Care. Processes and maintains records of residents of Copernicus Lodge and assists them with their specific needs as they relate to their accommodation.
In collaboration with the DOC and Administrator, the Placement and Resident Services Coordinator works as part of the multidisciplinary team and uses a wide range of highly skilled casework, group-work, and community organization techniques to:

  • Assess resident and family dynamics and needs.
  • Provide emotional support to residents and family members.
  • Act as the resident's liaison in the community, making referrals whenever possible.
  • Rectify issues that impact both the residents and other departments (i.e. non
- payment of LTC accommodation fee, completion of government documents, contacting PGT, etc.)

  • Ensure adherence to all applicable laws and regulations including the Resident's Bill of Rights, Collective Agreement, where applicable and Copernicus Lodge's Mission Statement and policies.
  • Communicates with TCLHIN, the Administrator and the DOC throughout the admission process
  • Maintains information package that meets the ministry's criteria and regulations and internal policies and procedures.
  • Coordinates and facilitates tours to potential and newly accepted residents and their family members/POAs
  • Monitors waiting list and provides the Administrator with monthly updates
  • If the Family Council so requests, shall act as the Family Council assistant.
  • Acts as an advocate for residents, lobby for solutions to problems directly affecting residents and develop prevention and intervention programs to meet their needs

Applications / Tours

  • Handles all telephone calls and correspondence relating to admissions, discharges and any inquiries relating to placement.
  • Ensures the in-house tours of prospective applicants are completed throughout the week and on weekends by the Placement and Resident Services Coordinator or by the Manager on-duty on weekends. This will ensure appropriate marketing of vacant beds and the home in general.

Admission/Discharges

  • Completes all aspects of the admission process including signing of the Long-Term Care agreement and all other applicable paperwork.
  • Reports LTC Bed vacancies to TCLHIN within 24 hours.
  • Coordinate with the Accounting department to ensure that the home receives the first month's payment.
  • Inspects and coordinates the cleaning or preparation of vacant rooms with Environmental Services and DOC or designate.
  • Issues LTC room keys.
  • Ensures completed health assessment forms and functional assessment forms for Long-Term Care admissions are submitted to the Nursing Administrative Assistant at least one day prior to admission so that she can set up the medical chart.
  • Notifies all necessary departments of all Long-Term Care admissions, discharges and transfers.

Communication/Documentation/Record-keeping

  • Keeps resident files up to date including the documentation of all accommodation discussions had with residents and/or their family/substitute decision maker.
  • Handles resident housing concerns and refers all other concerns to the appropriate department manager as necessary
  • Updates the Copernicus Lodge General Information sheets and Long-Term Care information booklet as changes occur.
  • Maintains appropriate statistical and documented information relative to all issues and dealings with the TCLHIN. This includes maintaining detailed statistics on the period of time it takes TCLHIN to fill each LTC bed vacancy.
  • Completes and maintains a checklist to ensure a Letter of Authorization is received from TCLHIN following a resident's admission into LTC. Follows up in a timely manner with CLHIN on all Letters of Authorizations not received.
  • Archives discharged files and other outdated information in a timely manner.
  • Maintain IAR Consent Form information
  • Develops and interprets the philosophy, objectives, policies and procedures and standards of services for the Placement Services.
  • Prepares the Placement and Resident Services Coordinator Annual Report.
  • Maintains a good knowledge of the Ministry of Health standards and criteria for resident care, programs and services in LTC Facilities.
  • Maintains an updated and current file system including a computerized generated system for all standardized forms, letters used in the placement/discharge process.
  • Oversees the Long-Term Care bedside table key boxes. Ensures that duplicate keys are ordered as necessary, and payment is received to cover the cost for replacement of lost/mis
