The Centre for Family Medicine is seeking a FTE Social Worker/Psychotherapist.


The Social Worker/Psychotherapist will provide comprehensive and coordinated mental health services to Centre for Family Medicine rostered patients and will function in close collaboration with physicians and other inter-professional health care providers within the Family Health Team.


As a vital member of our patient focused team, the social worker/psychotherapist will provide clinical services and will practice to their full scope of practice as defined by their regulatory College.


Duties/Tasks:


  • Provide short term counseling services to patients referred by family physicians
  • Experience working with individuals 12 years of age and older, couples and families
  • Facilitate mental health related groups and workshops, as required
  • Effectively complete assessment, treatment and management of a wide range of mental health issues
  • Make referrals to and liaise with other professionals on the interdisciplinary team and within the community
  • Collaborate with physicians and team members to collectively manage the needs of patients.
  • Collaborate with community resource organizations as needed.
  • Communicate effectively in written and verbal form on behalf of patients. This includes communicating with primary health care providers, other team members and community care providers as well as charting case notes in EMR.

Qualifications:


  • Masters degree in Social Work or Psychotherapy from a registered educational institution
  • Registration with applicable College and proven evidence of liability insurance
  • Minimum 3 years experience is an asset

Competencies:


  • Strong experience in delivering clinical counseling /therapy
  • Excellent problem solving and interpersonal skills
  • Excellent verbal and written communication skills
  • Demonstrated ability to work within teams
  • Demonstrated ability to work with ambiguity and mínimal supervision
  • Experience in supervising practicum students is considered an asset
  • Experience working with Autism Spectrum Disorder or using the neurodiversity approach is an asset
  • Ability to follow protocols, demonstrated knowledge of CFFM policies and protocols
  • Adhere to the Health and Safety policy and procedures
  • Participate in continuous learning
