About Us:

Reconnect Community Health Services is a not-for-profit health service organization in the west end of Toronto. Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. Reporting to the Director, Community Support Services and Performance Improvement, the Program Manager will ensure the delivery of high-quality services, staff orientation and supervision, quality management and performance improvement.

What You'll Do:

You will be entrusted with a wide range of responsibilities, including:

  • Ensuring that the assigned programs meet established objectives and standards;
  • Developing and overseeing program processes, as well as setting and monitoring achievement of program goals;
  • Overseeing and reporting program evaluation and outcome measurement results to the Director;
  • Recommending program improvement and expansion initiatives to the Director and participates in their development;
  • Monitoring all program client recording and statistical reporting to ensure that information is up to date, accurate and complete in the client information system and other files;
  • Monitoring and reviewing the implementation of existing departmental activities to make optimum use of staff, facilities and resources;
  • Evaluating the performance of staff according to policies and procedures, including performance reviews and evaluations;
  • Participating in all leadership development implementation activities for staff and themselves;
  • Providing support to hiring, disciplinary and grievance processes and conducts yearly performance appraisals of staff;
  • Conducting and documenting formal supervision meetings with staff.

What We're Looking For:


  • University degree and/or Community College Diploma in social services or related field (an asset);
  • Minimum 3 years experience in formal supervisory roles;
  • Demonstrate superior ability to supervise, coach, mentor and engage staff;
  • Superior ability overseeing the daytoday operations of assigned programs;
  • Ability to manage multiple priorities simultaneously with pressure of changing demands.
  • Selfdirected and independent, but also experienced in working effectively as a member of a team, which includes internal and external stakeholders;
  • Above average written ability; ability to analyze written reports, complex documents and publications;
  • Familiar with relevant legislation (e.g., Personal Health Information Protection Act (PHIPA), Employment Standards Act, etc_._);
  • Effective attention to detail and a high degree of accuracy;
  • High level of integrity, confidentially, and accountability;
  • Sound analytical thinking, planning, prioritization, and execution skills;
  • Excellent communication skills, both verbal and written;
  • Applied computer skills (Microsoft Office, Client Information Systems, Web browsing).

Special Requirements:


  • Criminal background check;
  • Must be willing to work flexible hours including evenings, weekends, statutory holidays, as required;
  • Must be willing to participate in an oncall rotation.
