Position Overview:


The Connected Care Halton Ontario Health Team (CCHOHT) is one of the first OHT teams in the province that is working to implement a new model of organizing and delivering health care that better connects patients and providers in their communities to improve patient outcomes.

Ontario Health Teams (OHTs) are part of the Ministry of Health's broader health system transformation strategy to enable patients, families, communities, and providers to work together to achieve better population health outcomes.


The CCHOHT represents a partnership of health care providers in the Halton Hills, Milton, and Oakville communities committed to better population health, better patient experiences, reduced costs, and a better care team experience.

The CCHOHT is guided by a Collaborative Committee with equal leadership representation from Acclaim Health, Home and Community Care Support Services, Halton Healthcare, Halton Region, as well as physician representations (primary care from within the geographical areas of Halton Hills, Milton, and Oakville and Specialist representation) and the chair of our Patient, Family and Caregiver Advisory Committee.

This position is funded by and will report into leadership of the Connected Care Halton Ontario Health Team. Halton Healthcare is supporting this recruitment as a partner of the CCHOHT.


This is what you'll be doing:

Working with the Director - Connected Care Halton Ontario Health Team, this position will be responsible for coordinating and managing a wide range of projects and activities related to the CCHOHT strategy and operations.

This position will provide project management support to all work stream and group activities, as required. _Having a strong vision and passion to improve the health care system will be a necessary strength._

Working with the direction of the Director, Connected Care Halton OHT, this position will conduct project management activities, including but are not limited to:
Project Management and Organizational Skills

  • Works in a _selfdirected manner_ to manage a portfolio of complex projects across varying areas of expertise, as defined within the overall context of the CCHOHT strategy.
- _ Manages numerous activities of high priority, complexity_, _and _cross partner impacts_, which may span across functional areas; usually called upon to perform in multiple roles.
- _ Provide support to the work stream and working group co-leads_ and their designated project leads on various initiatives and projects by managing, developing, and evaluating methods and procedures that are efficient, effective, and utilize good project management and change management practices to support assigned projects.
- _ Strong attention to detail, thoroughness when analyzing_,_ and planning activities.

Problem Solving & Critical Thinking

  • Works with stakeholders and project teams to analyze projects for _needs assessment, system selection, and/or process _improvement opportunities;_ makes recommendations to improve service delivery and the reengineering of processes.
  • Works with stakeholder groups to _solve innovative business problems_ with available resources.
  • Employ a full range of organizational resources to gather information, involve others, and acquire timely endorsements and approvals.
  • Works on _unusually complex problems_ and is sought as a resource for resolution of unique or highly complex business problems by providing highly innovative, creative, and integrated solutions that will achieve quality and work process improvements.
  • Functions at and utilizes a _high level of autonomy in determining objectives of assignment to achieve desired results_, ensuring time management, delegating assignments, scheduling/prioritizing activities, and determining, monitoring, and reviewing goals, performance, and expectations.
  • Supports and monitors key deliverables to ensure milestones are met.
  • Develops and documents related policies and procedures, as required.
  • Reviews own work upon completion for adequacy in meeting milestone objectives and achieving longterm desired organizational results.
  • Demonstrated knowledge and experience with health care system structure and design across the continuum.
Stakeholder Engagement, Change Management and Communications

  • Obtain _stakeholder input and feedback through various methods_ and individual consultation to ensure stakeholder perspective is understood and represented in all aspects of project codesign/development and implementation.
