_ Program Overview - Supportive Housing, Community Care_

  • The St. Hilda's Supportive Housing Program provides housing for seniors who have been street involved, and who may be experiencing mental health and/or addictions issues.
The program provides onsite psychosocial, primary care supports and services including:

intensive case management; harm reduction and addiction supports; therapeutic and supportive counseling; 24-hour staffing; assistance with activities of daily living; nursing services; life skills development; social and recreational activities; volunteer opportunities; and follow up support.

_What You Will Do_
- _

Provide comprehensive case management services; assessment, personal goal setting through the development of Case Plans, short and long-term planning, informal counselling, follow up supports, crisis intervention, service co-ordination, case conferencing, and referrals.

- _ Undertake needs assessments and develop case plans with individuals in the program._
- _ Assist tenants with mental health issues by providing a variety of day-today physical, emotional and social supports, life skills assistance, information and resources._
- _ Identify and address mental health issues connecting them with primary care and other resources and support them to utilize informal and formal support networks appropriately._
- _ Work closely with staff and clients on strategies that support clients with substance use and mental health issues to maintain their housing._
- _ Advocate on behalf of clients when required. _
- _Link clients to various community resources and facilitate appropriate referrals to other community support services when required._
- _ Ensure the coordination of services and a "continuum of care"._
- _ Liaise, consult, and collaborate with other program area staff and service providers. _
- _Monitor the housing challenges and ability of each resident to adhere to the policies and protocols of Supportive Housing Program and develop a team strategy for intervention when problems occur in accordance with housing first and eviction prevention principles._
- _ Complete statistics/reports and maintain all records regarding program/activity enrollment. _
- _Ensure that the anti-racism objectives and community development principles of the organization are achieved in the program. _
- _Gather statistical and narrative information required for the Service and organization._
- _ Participate in ongoing training and education._
- _ Participate in regular supervisory sessions and annual performance appraisals._


_What You Bring to the Team_
- _
BSW, or equivalent Bachelor level degree._
- _ 3+ years of experience in mental health and harm reduction, housing help, and working with those who have experienced homelessness._
- _ Effective September 7, 2021, all newly hired employees will be required to provide proof that they have completed an approved course of vaccination against COVID-19 at least 14 days prior to commencing employment or provide documentation to support an exemption based on grounds covered by the Ontario Human Rights Act (i.e. Medical exemption)._

- _ Demonstrated interpersonal skills, excellent verbal and written communications skills._
- _ Information and referral skills; and ability to effectively link client to supportive services._
- _ Ability to work from a client-centered approach._
- _ Comprehensive understanding and knowledge of the health sector and its related services._
- _ Excellent knowledge of community resources. _
- _Demonstrated ability to follow policies, procedures, and adhere to legislation._
- _ A thorough understanding of clinical practices and care models._
- _ Proven ability to facilitate presentations and workshops._
- _ Applied computer skills (MS word, excel, and Internet)._
- _ Ability to use case management software is an asset (Pirouette)._
