To provide a safe, structured, educational and developmentally appropriate home environment that promotes a sense of well-being, self-confidence, belonging and independence for the individual.

Collaborates with the individual, their families and other professionals to pro-actively promote opportunities for inclusion in both the community and the individual's home life.

Manages all aspects of the home environment including individual, the care team, home operations and finances.


The role includes the requirement to work in a stressful environment often dealing with individuals with highly challenging behaviours and in crisis situations.

The ability to function independently and frequently under pressure while managing emergency situations is an ongoing expectation. Evening and weekend work may be required.

Responsibilities and Accountabilities:


1. INDIVIDUAL CARE


Collaborates with the individual, the individual's family, Ministry, Clinical team (as appropriate), Social Workers and other professionals, and is responsible for the preparation of a Plan of Care / Personal Centered Plan for the individual.

Collaborates with direct Supervisors within the Agency in developing positive effective relationships with the 'external' team

Implements and monitors Plan of Care / Personal Centered Plans and provides assistance to individuals with
- development of their sense of independence
- teaching and building life skills
- including them in the greater community
- introduction to the workplace (where appropriate)
- development and evaluation of all individual goals and objectives with the assistance of the house team

Assists and directs staff with planning and providing service delivery to individuals, case conferencing (bringing the entire care team together to discuss possibly a new approach for the individual) and case management for individuals in residence


Co-ordinates with house team and individuals to develop a recreational activity calendar which provides a wide range of social and leisure activities that includes and promotes resources in the local neighbourhood and in the wider community.

Is responsible for managing "in house" staff ensuring that they are executing their responsibilities efficiently and effectively

Prepares individual for outings including weekend trips home where travel, clothing and medical supplies need to be planned and organized


Enhances the individual's potential (likes and dislikes) and their ability to meet their goals by creating a supportive positive environment.

Acts as a liaison with the community and promotes/encourages community involvement. Establishes a rapport with the community and community agencies. Acts as liaison with government officials and other professionals. Maintains open communication with resident's families and relevant ministries. Strongly advocates for individuals in care when they are not able to do so themselves.

Supports the individual in the learning of daily life skills (as appropriate) such as cooking, washing dishes, cleaning up after themselves, etc


2. HOME OPERATIONS


Identifies and is responsible for meeting both physical and program needs of the residence and deals with separate issues accordingly.

May address such needs to the Executive Director on an as required basis.

Ensures that high standards of service delivery to all individuals are implemented.


Supervises the house staff to ensure that they maintain, within community standards, a clean, tidy and safe home for individuals, other staff and visitors.

Manages the performance of all "house" staff

Ensures that individual care team is advised of any relevant developments that affect the individual or the home

Supervises all workers and monitors daily operations within the residence.

Assists the Human Resource Team in recruiting selecting worker; as well provides guidance to all workers on policies, procedures, techniques or other matters arising in the residence.


Medication:

  • Ensures all medication prescriptions and "MAR" sheets are current
  • Ensures medication is picked up or delivered, is accurate in quantity and dosage, is labelled with appropriate date, signed off and stored in the home's safe.
  • Ensures that any worker administering medication has completed PCSL'S (medication administration procedure) and has signed current MAR sheet.
  • Ensures that all practices in this area are in compliance with the Agency's Policies and Procedures.
Prepares or assists with preparation of all meals for individuals using Canada's Food Guide as a reference. Quality and appropriateness of diet is important.

Prepares clothing inventory for individual

Purchases personal items for individuals or delegates the purchase of these to others.

Manages, encourages and monitors staff to ensure that their responsibilities are executed properly to ensure the home is saf
