Overview


The Case Management Coordinator provides leadership in case management activities for all people we support at Ray of Hope - Secure.

Through using information provided upon intake and assessment, the Case Management Coordinator, in collaboration with other team members develops individual case management plans and communicates such plans with all stakeholders.

As part of case management planning, the Case Management Coordinator will develop reintegration plans for the people we support. They will assess and evaluate the progress of goals as set out in the plan.

Specific Responsibilities

  • Provide and review screening, intake, and assessment for young persons to support in development of Case Management Plans
  • Develop Case Management plans for each young person based on the Risk Needs Assessment working in collaboration with Young Person, Probation Officers, Caregivers, Youth Care Workers, Social Worker, etc
  • Complete and review Case Management reports
  • Coordinate and actively participate in case management meetings
  • Inform, plan, and discuss young person's case management plan with Youth Care Workers, WRDSB Teachers, Chaplain, and Supervisors, etc.
  • Evaluate progress of goals as set out in Case Management Pl.
  • Provide reintegration services to youth
  • Maintain positive relationships with other Service Providers
  • Coordinate referrals with other professionals
  • Assist with the planning and implementation of program enhancements.
  • Provide crisis intervention services as required
  • Create and maintain young people's records in a confidential and professional manner
  • Compile caseload statistics and prepare program and client reports
  • Complete and maintain accurate records and reports regarding the young persons' history, progress, services provided and other requirement information

Qualifications:


Education

  • Completion of related postsecondary education (i.e., BSW, SSW)

Experience

  • 12 years' experience working with at risk youth and in Case Management planning.
  • Preferred: Experience working in youth justice.

Certifications

  • Must be and maintain accreditation as a Registered Social Worker (RSW) or Registered Social Service Worker (RSSW).
  • Understanding and Managing Aggressive Behaviour (UMAB) Certificate (willing to train)
  • ASIST training an asset (willing to train)

Knowledge/Skills/Abilities required to be successful in this position

  • Familiar with policies of Ministry of Children, Community and Social Services
  • Youth Justice Services, as well as applicable legislation governing (Child, Youth and Family Services Act, Youth Criminal Justice Act)
  • Proficient in reading, writing, speaking and comprehension in the English language.
  • Proficient in the Microsoft Word, Excel, Database

Requirements:


  • Vulnerable Sector Check/Police Check
  • Valid, unrestricted Class "G" Ontario Driver's licence
  • Ministry of Transportation Drivers Abstract through Service Ontario
  • Employment Medical Form

Physical Demands

  • Standing (20%), sitting (60%) and walking (20%).
  • Frequent use of mental capabilities and concentration.
  • Possible exposure to hazards such as harassment, physical or verbal aggression or aggressive behaviours, witness to harassment or violence, and similar variables as displayed by the persons we support.
  • Possible risk of vicarious trauma.
  • Infrequent exposure to environmental hazards, including Hot/Cold and regular environmental variables
  • This position requires the person to consistently work with and around others.
  • The incumbent may be required to physically intervene with male youth utilizing an approved restraint program.

Working Contacts

The Case Management Coordinator will have significant contact with youth, family members of the young person as well as with community professionals such as Probation Officers, medical professionals, external counsellors, etc.

They will also have contact with internal parties such as Youth Care Workers, Shift Supervisors, and other members of the facility staff.
