Position Summary:


The Behavioural Supports Lead (RPN) consultant role will lead a mobile support team of Resident Care Assistants (RCA) to enhance the care and support of older adults who live and cope with various types of dementia, delirium, mental health, addictions, other neurological conditions and those who experience acute behaviours changes.

The role will demonstrate the Provincial BSO Framework of Care and Competencies.

The position is responsible for planning, organizing, and facilitation of a Mobile Behaviour Support Team, supporting Long-Term Care Homes and the broader Behavioural Support Ontario (BSO) Network within the Central West Ontario Health Region.

The position will optimize engagement of residents and families' experience including oversight and supervision of a team of BSO RCA's.


The lead will work independently and collaboratively as part of the interprofessional care team by providing supports to Residents, Families and staff; to maintain safety, competency and improve resident quality of life.

The Behavioural Supports Lead (RPN) formulates interventions, collaborates with care partners to navigate the care system to develop person-centred support for residents through a referral-based system.

Behavioural Supports Lead (RPN) provides education to improve staff understanding and competency based on research, risk management, evidence-based, patient-centered care and practice.


Specific responsibilities include:


  • Working collaboratively with other Long-Term Care Homes within the Central West Ontario Health region and the BSO Network to develop, implement, and monitor a Behavioral Supports Program that ensures individual quality care, manage dysfunctional behaviors to reduce disruptive behaviors by therapeutic communication and environmental manipulation.
  • On a referralbased system, coordinate the assignment, scheduling, and deployment needs of the Mobile RCA Team to assess, support various personal expressions experienced by individuals with dementia, delirium, mental health, addictions, other neurological conditions, and those who experience an acute behavioral change who can be best served by a behavioral support team.
  • Ensure residents' needs are based on the residents' preferences, strengths, social and personal resources, interest, health status and functional, intellectual and cognitive limitations.
  • Ensure the care provided to each resident meets social needs including relationships with family and friends; recreation and leisure pursuits; meaningful activity and community interaction.
  • Implement care plan and crisis manage strategies and behavioral interventions.
  • Ensure residents in the Behavioural Supports Program receive detailed psychosocial, activities and behavioral assessment of daily living, mental status and sensory stimulation needs.
  • Design, coordinate programs to provide skill, knowledge and training to support staff in meeting the needs of the residents.
  • Ensure that the environmental design for cognitive impairment focuses on providing a comfortable, nonthreatening environment with attention provided to reducing stimulation and providing safety and security.
  • Other duties as required.

Qualifications:


  • Must possess and supply annually a current certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse.
  • Minimum twoyears work experience with residents in Long-Term Care.
  • Training in (or willingness to participate in training), P.I.E.C.E.S, GPA Coach, Mental Health First Aid, and Montessori based programming and considered an asset.
  • Familiarity with RAI documentation including assessments, RAPs and care planning functions.
  • Valid driver's license and access to reliable vehicle required.
  • Demonstrated commitment to continuous learning, professional development, quality improvement and change initiatives.
  • Sound understanding of and commitment to the principles of Behavioural Supports Ontario.
  • Familiarity with the Regulated Health Professions Act, Occupational Health and Safety Act, WHMIS, and the Standards of Practice
