Mental Health & Justice Prevention Program (Full Time - 6 Months):


The Role and Opportunity:

CMHA Toronto's

Mental Health and Justice Prevention Program is seeking a

Case Manager. The Case Manager will work within team of case managers in east Toronto to provide community-based support and linkages to individuals with serious mental illness who are at significant risk of incurring criminal charges and/or at
_ _significant risk for apprehension by the police.

RESPONSIBILITIES

  • Outreach and assessment
  • Orientation to program and agency
  • Clientcentered engagement, service provision and interventions
  • Skill development and goal planning
  • Providing resources, information and advocacy
  • Maintaining a trusting, respectful and collaborative working relationship
  • Monitoring, evaluation and followup

SKILLS YOU BRING

  • Bachelors level education in Social Work/Social Science or equivalent
  • 3 years experience in community mental health and addictions
  • Ability to work with diverse communities
  • Ability to demonstrate a nonhomophobic and antiracism approach
  • Experience in case management
  • Second language would be an asset

A valid driver's license, a car and insurance coverage are required for this position.

Salary:
$50,578 - $62,730 + 4% in lieu of vacation
