Position Profile


This role provides routine care activities, support and assistance to people with medical conditions and/or functional limitations, and their families to carry out activities of daily living with the goal of supporting client independence and promoting optimal functioning.

The PSW works collaboratively with a variety of professionals in diverse care settings such as institutions or a home-care environment.

Reporting to a registered healthcare professional.


Key Activities and Responsibilities

  • Adhering to professional standards and established policies and procedures, assist clients with all activities of daily living (ADL'), including personal hygiene (showering, bathing); incontinence care; dressing and grooming; light massage and passive range of motion (ROM), ambulation, positioning and transferring, feeding by mouth, mouth care.
  • Assist with 'instrumental activities of daily life' (IADL's), including laundry and linen change, light housekeeping, meals and nutrition, and transportation planning.
  • Working under the general guidance of a registered healthcare professional, follow/implement the established care plan, observing and reporting any substantial findings and/or changes in patient/resident/client's behavior.
  • Provide skilled and compassionate care in a variety of environments (client's home, retirement home, community, long term care or hospital setting) including 1:1 care or shift work, assisting nursing staff or other team members by answering call bells
  • Companionship, social and emotional support. Accompany on errands and to appointments (via taxi or own vehicle).
  • Other duties as assigned by the supervisor or devised under a specific care plan

Skills & Qualifications

  • Personal Support Worker (PSW) certificate from an Ontario Community College Program OR a private career college in Ontario that is registered with and approved by the Ministry of Training, Colleges and Universities with successful completion of the final examination administered by the National Association of Career Colleges (NACC) OR a approved Ontario Board of Education PSW Program
  • Must have a minimum of 1 year Personal Support worker
  • Experience in Dementia or Alzheimer's preferred
  • Must be able to communicate fluently (written and verbally) in English. Fluency in French (written and verbal) would be an asset
  • Proof of current immunization for measles, mumps, rubella and varicella (MMRV), diphtheria, pertussis and hepatitis b
  • Proof of negative TB skin test within the past one (1) year or negative chest xray within the past five (5) years
  • Certificate or Equivalent Current CPR and First Aid Certification
  • A clear Criminal Record Check with Vulnerable Sector Screening within the last 12 months.
  • Ability to travel from home to home within the community
  • Commitment to work every other weekend
  • Strong verbal and written English communication skills, with ability to communicate in other languages an asset
  • Reliability and strong organization skills
  • Ability to safely lift up to 35 lbs.
  • Possess a valid driver's license is an asset, but not essential.
  • Possess a vehicle with valid auto insurance is an asset but not essential.
