Summary:

Reporting to the Patient Care Manager, Ambulatory Care Clinics, Medicine Program, the Program Coordinator provides leadership in the development and implementation of clinical processes and procedures to determine patient flow through the Provincial Program Home HPN/HEN.

The Program Coordinator works with the team to provide consistency in service and to evaluate and implement changes to increase efficiency and access for patient services.

Determines priority and admission of patients into the Program and manages any and all concerns raised. Works with the Physician Lead on ongoing Program development.


The Program Coordinator supports and collaborates with the Patient Care Manager to ensure the Provincial Program complies with clinical and corporate standards and processes, and develops and implements ongoing evaluation and quality improvement initiatives to ensure best practices and outcomes.

Coordinates project development for a broad range of HPN/HEN initiatives.


The Program Coordinator, in collaboration with the Patient Care Manager, provides work direction and ongoing consultation to a multidisciplinary care team and enhances partnerships with key stakeholders at all levels both internal and external to the organization.

Partners with Supply Chain to negotiate vendor contracts for equipment and services on behalf of the Patient Care Manager. Acts as a representative on industry committees and working groups as required.


Qualifications / Skills and Education:
Education

Master's Degree in a health sciences discipline and three (3) to five (5) years' recent, related clinical experience in a large, complex health care environment, or an equivalent combination of education, training and experience.

Skills and Abilities

Comprehensive knowledge of health care delivery systems.

Ability to facilitate nutrition related teaching to professional staff.

Ability to develop and maintain effective partnerships at all levels of the organization, and with internal and external stakeholders.

Physical ability to perform the duties of the position.


Duties and Responsibilities:

Assumes a leadership role in collaboration with the care team in the patient priority and admission into the Provincial Program.

Develops and implements processes/criteria that make the Provincial Program more effective. Assists the HPN/HEN Program care team to evaluate their care to ensure quality standards are maintained.


Integrates the Provincial Program with other internal and external/community programs and services that ensures the responsible use of fiscal and human resources, avoids duplication and compliments existing resources.

Works closely with the Program's Physician Lead on ongoing Program development with respect to patient services delivery and care.


Participates in vendor selection processes with the care team, meets with vendor representatives, negotiates contracts in collaboration with the Patient Care Manager and monitors vendor's adherence to specifications such as product quality and agreed upon timelines.

Liaises with Supply Chain regarding contract wording, and other considerations as required.


Manages patient concerns, including but not limited to concerns regarding equipment and/or service, on behalf of the Provincial Program through entire process from initial notification of concern through to concern resolve and patient follow up.

Thoroughly investigates each concern, and documents. May elevate to the Patient Care Manager as appropriate.

Coordinates patient transfer by working with the Clinical Nurse Leader Access and Flow regarding bed availability and timing.

Ensures care team complies with established clinical and corporate policies, standards ad procedures. Works with the team and Physician Lead to evaluate effectiveness of processes and identifies opportunities to enhance patient outcomes.

Monitors budgets, in association with the Patient Care Manager, assisting them to investigate variances and discrepancies. With Patient Care Manager support, initiates corrective actions and identifies trends for future budget planning. Seeks opportunities to reduce costs, and prepares rationale and justification for budget request.
