JOB SUMMARY:


Provide comprehensive case management services for individuals with severe and persistent mental illness, individuals who are newly diagnosed with a psychotic disorder, or to provide support to youth transitioning from the youth sector to adult sector.

Services are provided in the home or at community locations convenient for the client.

Provide proactive outreach, facilitation and assistance to meet client basic needs, assist with navigation through the health care system and support linkages to appropriate community resources.


QUALIFICATIONS:


EDUCATION:


  • A diploma or degree in human servicerelated discipline.
  • Demonstrated thorough comprehensive knowledge of severe mental illness and treatment, and its impact on consumer/survivors and their families, which includes the effects of poverty and marginalization.

KNOWLEDGE/EXPERIENCE:


  • Minimum 23 years experience working with the severely mentally ill population, preferably in a community setting.
  • Personal and/or familial exposure to mental illness would be advantageous.

SKILLS:


  • Skills in counseling, addictions, housing, community outreach, concurrent disorders and general aspects of physical health are an asset.
  • Must demonstrate excellent communication skills, both verbal and written, and knowledge of local community resources.
  • Sound understanding of concurrent disorders is an asset
  • Computer literacy is required
  • French language is an asset

SPECIAL CONDITIONS:


  • Must provide own transportation and be able to travel throughout the Niagara region to other offices, work locations, or sites as authorized by the Corporation for business reasons. Employees who use their personal vehicles for approved Region business shall ensure they are adequately insured with their personal insurance carrier. It is the employee's responsibility to possess and maintain a valid driver's license of the appropriate class to carry out their job in accordance with Corporate Policy.
  • Support clients in their travel to locations in the community when it is relevant to the client's recovery plan as per program policy (which allows the option of staff transporting clients in their personal vehicle)
  • In accordance with Corporate Criminal Records Check policy, the position requires the incumbent to undergo a Criminal Records check, including vulnerable sector screening, and submit a Canadian Police Clearance Certificate prior to commencing employment
  • Be available to work flexible hours including weekend and/or evening hours as required
  • Vaccinations as recommended in Public Health policy
