Position Profile:


The Housing and Aftercare Supervisor is a key leadership role that is focused on supporting the most marginalized and vulnerable youth across the Region of Peel.

The incumbent will empower and guide youth towards achieving their housing goals by providing ongoing support with housing resources and navigation, advocating on behalf of the youth, and establishing and maintaining relationships with landlords, housing providers and community partners to expand housing options for youth.

Upon graduating from the Transitional Housing program, the incumbent will provide aftercare services and support to the youth.


Role Overview:


  • Housing: _
  • Meet with youth to formalize their housing plan and to identify housing options that align with the youth's needs.
  • Work with the young person on their housing search, landlord engagement plan, and budgeting.
  • Provide housing support by facilitating housing related workshops and administering rent subsidies
  • Support the youth in attending housing appointments and act as their liaison to ease barriers to housing, monitor quality control and screen for suitability related to health and safety standards.
  • Build and establish relationships with landlords and expand our landlord/housing registry.
  • Work with partners to share weekly affordable housing listings.
  • Source new partnerships and supplies for affordable furniture and household items to assist the youth in transition.
  • Ensure that the continuity of appropriate admission, service, and discharge procedures are in line with the agency's philosophies and complaint with the Region of Peel's Transitional Housing Standards.
  • Aftercare: _
  • Provide aftercare services and support to the youth upon graduating from the Transitional Housing program through housing maintenance, rapid rehousing support up to two years posttransition and all other supports to ensure they maintain their housing.
  • Support the maintenance of housing, employment, education, social integration as well as financial supports if appropriate to assist with the youth's continued independence and prevent homelessness recurrence.
  • Engage in case conferencing with the young person and their circle of support to ensure appropriate levels of supports are in place to be successful with their housing.
  • Conduct checkins and site visits, complete community referrals, engage in landlord mediation and all other supports to ensure they maintain their housing.

  • Compile statistical information and prepare monthly reports as required for program evaluation and development.
  • Participate on committees and task forces as required with other agencies addressing issues facing youth.
  • Create a safe and secure setting for all youth accessing services at the transitional program.
  • Ensure that professional standards are maintained as related to client documentation and case management.
  • Ensure that the facility meets health and safety standards.
  • Foster a positive and supportive working environment.
  • All other duties as required in the successful management of program operations while partnering with all stakeholders in support of organizational success.

Qualifications:


  • Degree in Social Services or in a related field or an equivalent combination of education and/or experience.
  • Experience within the social service sector supporting youth experiencing homelessness with securing and maintain housing.
  • 34 years of experience along with 1year supervisory experience working within the social service sector, preferably in a shelter/residential setting or equivalent combination of education and/or experience.
  • A leader with excellent communication, presentation, organization and interpersonal skills with a passion for supporting the most vulnerable and marginalized youth in our community.
  • Flexibility to work in a 24hour, 7 days a week operation, while supporting an oncall rotation.
  • Knowledge of resources and supports for youth
