KEY FUNCTION:


Work as part of an interdisciplinary team to provide addictions support to clients accessing services within Addictions Services, including the Rapid Access Addiction Medicine (RAAM) clinic, Addictions Medicine Consult Team (AMCT), Withdrawal Management Services (WMS), Safe Beds (STSB) and the Outpatient Addictions and Gambling Service (OAGS) and provide referrals to community programs for ongoing support.


REPORTING:

Under the general direction of the Regional Lead, Addictions Services.


DUTIES:


  • Complete assessments with individuals and provide referrals as appropriate.
  • Work collaboratively with addictions physicians and client in determining appropriate treatment modality.
  • Provide navigational support to individuals accessing services throughout trajectory (intake, during treatment and on discharge from clinic including transfer of care plan).
  • Ensure flow in the clinic and that individuals are being connected with community resources and back to their primary care provider.
  • Perform data collection and information transition to other parts of the healthcare system at time of discharge.
  • Assist in development of referral pathways.
  • Provide educational support with prescriber lead and coordinator to train individuals in the hospital and in the community.
  • Attend community of practice meeting and other meetings as required.
  • Ensure equitable access to and completion of patient satisfaction tool.
  • Determine and align improvement projects with HSN's Strategic Plan; monitor and adjust to achieve goal outcomes.
  • Evaluate patient care and satisfaction on an ongoing basis and formulate solutions for improvement.
  • Educate and promote health, safety and wellness in the work place.
  • Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
  • Perform other duties as required.

QUALIFICATIONS

EDUCATION AND TRAINING:


  • Minimum of a two (2) year Community College Diploma in a health or social sciences field.
  • Currently eligible to obtain the International Certified Alcohol and Drug Counsellor (ICADC) certificate.
  • Current Basic Cardiac Life Support (BCLS) and First Aid certificate required.
  • Ministry of Labour "Worker Health and Safety Awareness in 4 Steps" training certificate is required.

EXPERIENCE:


  • Minimum of six (6) months' experience working in an Addictions Services department within a health care environment.

KNOWLEDGE/SKILLS/ABILITIES:


  • Demonstrated ability to work effectively with men and women in behavioural crisis.
  • Demonstrated ability in completing assessments, including treatment assessments such as Global Appraisal of Individual Needs (GAINS).
  • Demonstrated ability in navigational support and providing referrals to community programs for ongoing support.
  • Demonstrated ability to provide education to groups.
  • Demonstrated training, experience or utilization of lean methodology for process improvement.
  • Demonstrated ability to independently identify issues, plan improvements, measure success and continue improvement.
  • Demonstrated excellent computer skills with proficiency in Microsoft Office software (e.g. Word, Excel, Power Point, and Outlook) and patient information systems.
  • Demonstrated superior interpersonal and communication skills, both written and verbal.
  • Demonstrated ability to manage routine correspondence, multiple tasks/projects, diversified workload and rapidly changing priorities and challenging deadlines.
  • Demonstrated discretion and maturity when handling confidential information.
  • Demonstrated commitment to the safety of coworkers and patients.

PERSONAL SUITABILITY:


  • Demonstrated commitment to addictions recovery, rehabilitation and use of harm reduction strategies.
  • Successful Vulnerable Sector check is required.
  • Demonstrated commitment to ongoing professional development.

  • Ability to meet the physical and sensory demands of the job.
