The Service Coordinator will provide direct support to clients.

They will perform administrative tasks related to the operation of programs and will assist/liaison with staff for the implementation and maintenance of all services.


ACCOUNTABILITIES

Client and Family Centric Care Delivery

  • Plans, organizes, and schedules the service delivery for clients including such services as homemaking, personal care, medication assistance, grocery shopping, transportation, and security checks.
  • Completes client intake for the Inhome Support department, receives referrals through HPG as needed including prescreener assessments.
  • Establishes a working rapport with clients that supports their dignity, selfdirection and that facilitates their activities of daily living (ADLs) and medical needs.
  • Supports clients with medication reminders related to taking their medications at the scheduled time(s) and in accordance with their physician's instructions, as required.
  • Complete Security checks and procedures related to safety.

Planning and Organizing

  • Schedules staff to accommodate new client referrals or to change from existing schedules, and to backfill staff absences.
  • Reconcile and complete payroll information for PSWs.
  • Inputs and maintains case notes and regularly reviews clients' Care Plans, complete documentation.
  • Inputs client information and service data into client data base (CIMS software), hard copy files and keeps additional manual records as required.
  • Creates, maintains, and updates client files.

Customer Services

  • Refers clients, caregivers, and professionals to appropriate program & services at Lumacare and/or other community resources as required.
  • Manage inquiries by providing information to callers and direct phone calls when necessary.
  • Support department coverage including Home at last, Meals on Wheels, and Reception, as needed.
  • Fulfills oncall duties as required.

ROLE REQUIREMENTS

  • Social Service Worker diploma or a diploma related to healthcare or support services is required.
  • Experience in staff scheduling and in payroll required.
  • Experience working with vulnerable older persons and older adults with mental health issues, addictions, and other special needs.
  • Knowledge of the Personal Health Information Protection Act, Accessibility for Ontarians with Disabilities Act, Home Care and Community Services Act and other relevant legislations of Ontario.
  • Ability to intervene and manage crisis situations.
  • Knowledge of the Inter-RAI CHA assessment tool and of CIMS/HGP is an asset.
  • Strong organizational and time management skills.
  • Basic knowledge of medications and health and wellness in general.
  • Excellent verbal and written skills in English language; proficiency in other languages such as Italian and Spanish an asset.
  • Valid Ontario Driver's License and access to a vehicle.
  • Immunization for tuberculosis, chickenpox, mumps, and hepatitis B.
  • Current First Aid & CPR Certification.
  • Current Vulnerable Sector Police check required completed within last 12 months is required.
  • This position requires you to be fully vaccinated against COVID19 and you must provide Lumacare with proof of vaccination prior to employment.
