Description

As part of Siloam Mission's mandate to be a connecting point between the compassionate and less fortunate, the job involves providing case management services to clients, with a focus on empowering them to make decisions, set and achieve goals, and maximize their self-determination.

The responsibilities include assessing clients' skills, strengths, and development needs, acting as an advocate for clients' needs, maintaining client records, monitoring and communicating client progress, providing tenant and eviction prevention services, helping residents identify vocational interests, attending meetings and training, and performing other duties as assigned by the Supervisor.


This position is responsible for coordinating and delivering support services to the clients by assisting them to achieve their goals and reach personal potential.


KEY DUTIES & RESPONSIBILITIES

  • Provide case management services to clients, helping them to maximize their ability for selfdetermination by supporting and empowering them in decisionmaking and assisting with goal setting, planning and follow through of identified goals;
  • Assess clients with relevant skills strengths and development needs;
  • Act as an advocate for the needs of the clients of the Madison;
  • Maintain client records in a timely manner;
  • Monitor, evaluate and communicate progress of clients with the client themselves, team members and supervisor on a regular basis;
  • Provide tenant and eviction prevention services;
  • Assist residents in determining their strengths and interests and pursuing vocational related activities;
  • Attend biweekly team meetings, attend staff meetings as required;
  • Participate in all mandatory training;
  • Other duties as assigned by Supervisor;

EDUCATION & EXPERIENCE

  • Postsecondary diploma or degree in related fields or a combination of education and experience will be considered.
  • Experience is case management in the social services field with focus on assessment, service planning, evaluation, follow up, review and closure.
  • Understanding of systems that our residents participate it. Such as EIA, home care, healthcare, supportive agency's etc. Experience working with those that have mental health illness and or addiction Assist, Mental health first aid, NVCI and first aid are encouraged but can be trained on site.
  • Valid Class 5G driver's license and access to a vehicle in good condition and willing to drive clients to appointments.

KNOWLEDGE/SKILLS/BEHAVIORS

  • Excellent human service planning skills and ability to engage a broad spectrum of community members in planning efforts;
  • Strong background in working with lowincome individuals with addiction, mental, physical, and developmental disabilities;
  • Exceptional skills in conflict resolution;
  • Experience with HIFIS;
  • Knowledge of Microsoft office suite;
  • Excellent interpersonal, communication, and organizational skills, as well as a genuine interest in working with marginalized persons;
  • Capable to work with a diverse staff team and promote inclusion and cultural awareness in the workplace;
  • Good understanding of colonization and its impact on Indigenous peoples;
  • Certificates in NVCI, ASIST, CPR First Aid, Mental Health First Aid; preferred Experience working with HIFIS4, is an asset;
  • Demonstrated knowledge of Cultural Awareness, Anti-Oppressive Practice, Trauma Informed best practices;
  • Knowledge of community resources for homelessness, health, mental health and substance use; and harm reduction;
  • Ability to communicate appropriate professional boundaries;

RELATIONSHIP

  • Interact daily with the people we serve.
  • Interact daily with interdepartmental staff and management.
  • As needed with outside departments and agencies.

WORKING CONDITION

  • Hours of work: 8am 4:30 pm or predetermined by management;
  • Office environment;
  • Part of the oncall rotation;
  • Occasional weekends or evenings, determined by need;
  • Hazardous conditions could include potential violent outbursts from community members toward staff or other community members.
