Position Summary
Required

  • Current registration with a regulated Health professional body in good standing,
  • Baccalaureate degree required,
  • Minimum 3 years' recent experience in geriatrics and/or emergency services,
  • BCLS required,
  • Completion of a recognized and accredited Gerontological certification required within eighteen (18) months of hire,
  • Minimum of three (3) years Acute Care experience (ED, Medicine, Surgical) preferred,
  • Demonstrated ability to develop and maintain collaborative working relationships with the interdisciplinary health care team and community partners including but not limited to (LHIN, LTC facilities, etc.),
  • Demonstrated ability to develop and deliver educational programs to patients/families, the health care team, and community partners,
  • Experience with electronic nursing documentation,
Preferred

  • Completion of Masters Degree in Education or other Healthcare field,
  • Gentle Persuasive Approaches (GPA) course,
  • Non-Violent Crisis Prevention Intervention (NVCPI) certification,
  • Knowledge of principles of adult education,
Duties

  • Provides consultative support to the Emergency Department Physicians and interdisciplinary team in the assessment and treatment of geriatric patients in the Emergency Department,
  • Screens, assesses, plans, and coordinates care for high risk elderly patients that present to the Emergency Department,
  • Serves as an advocate for elderly patients and their families while in the Emergency Department,
  • Acts as a coach, mentor, resource and positive role model for team members and students,
  • Complete comprehensive assessments and identify the needs of elderly patients in the ED, with the goal of avoiding unnecessary hospitalizations and to safely discharge patients to the community,
  • Facilitate timely and safe discharge of elderly patients from the ED and to link them to community resources,
  • Communicate assessment findings and needs of admitted patients to members of the receiving unit's interdisciplinary team
  • Promote a model of best practice in Elder Care nursing and an elder friendly philosophy of care within the ED
