The Salvation Army Toronto Grace Health Centre has immediate opening for full time Personal Support Worker 12 month contracts in our Specialized Care Centre, Scarborough Location.

Personal Support Worker

TEMPORARY FULL TIME 12 MONTHS

12 HOUR SHIFTS (Days/Nights/ Weekends)

Hourly Rate:
$22.317-$ in accordance with the Collective Agreement)

Position Summary and Responsibility:

The Personal Support Worker (PSW) is a member of the inter-professional team who works in collaboration with team members in the provision of patient care following the principles of Patient Centred Care and the Mission, Vision and Values as well as Policies, Procedures and Standards of Care of The Salvation Army Toronto Grace Health Centre (TGHC).

We are looking for a highly motivated individual and works collaboratively with the interdisciplinary team. This position will report to the Patient Care Manager.

Summary of Responsibilities:

Assists patients with Activities on the Daily living (i.e. assistance with meals, patients' transfers and repositioning, including the use of mechanical lifts as per policy as well as oral hygiene & toileting).
Performs activities or tasks delegated by a Registered Nurse (RN) or Registered Practical Nurse (RPN) in compliance with Regulated Health Professions Act. RNs or RPNs will be responsible for the ongoing supervision of the PSW in the performance of such tasks.
Ensures a safe environment for clients and staff by reporting and documenting unsafe conditions such as broken equipment as well as any patient behavioural, physical or cognitive changes.
Reports safety concerns ( including environmental hazards) immediately to the Patient Care Manager, Registered Nurse, Registered Practical Nurse, Occupational Health Nurse or administrator on call, as applicable
At mealtimes ensure that: food tray tables are cleared and clean to receive trays, patients are properly set up for their meal time, assist patients with the consumption of their meal as needed and load food trays onto the racks after mealtime is over.
Make beds and assist Environmental Services staff with patient personal laundry
Responsible for isolation bag removal and dirty linen bag removal from patient rooms
Clean and declutter patient refrigerators
Documents and maintains related patients' records and progress notes as per TGHC policies
Attends Transfer of Accountability meetings upon arrival to the assigned shift
Attends daily Patient Safety Huddles and education sessions as applicable
Perform other duties as required as assigned by the Patient Care Manager

Qualifications:

Successful completion of a PSW program from an Accredited Institution ( i.e. by the Ontario Ministry of Training, Colleges, and Universities)
Current Basic Cardiac Life Support (BCLS) Certificate
Behavioural management training ( i.e. PIECES. Ufirst
CPI) is considered an asset
Experience working with persons with Dementia or cognitive impairment
Flexible, adaptable to changing work environment
Communicates effectively in both oral and written English, excellent interpersonal skills
Demonstrates impeccable work ethic
Exercises good organization skills and take initiative to complete assigned tasks accordingly
Must have the physical strength, flexibility and endurance required to perform the job, as determined by the physical needs assessment of the position conducted by Employee Health Services
Demonstrates good understanding of Patient or Person Centered Care principles or values
Excellent knowledge and understanding of Infection Prevention and Control protocols for patient facing staff
Excellent attendance record.

**In order to be eligible for employment at The Grace

Specialized Care Centre, all new hires must have received at least two doses of a COVID19 vaccine approved by Health Canada prior to start date, and submit evidence of such.
